Description
INTENT TO SOLE SOURCE NOTICE This is not a request for quotes or proposals. VA Medical Center Altoona has a need for services to furnish all necessary storage facilities, delivery, installation, transportation, training, labor, parts, material, reports, etc., to provide storage, delivery, installation, pickup, repair and maintenance of VA owned Durable Medical Equipment (DME), to VA Beneficiaries at their residence located throughout the geographical regions, which includes, but is not limited to, Pennsylvania counties of Bedford, Blair, Cambria, Centre, Clearfield, Clinton, Elk, Huntingdon, Indiana, Jefferson, Juniata, Mifflin, and Somerset for the James E Van Zandt VAMC, 2907 Pleasant Valley Blvd, Altoona, PA 16602. DME includes but is not limited to, VA owned hospital beds, mattresses, side rails, wheelchairs, Hoyer lifts, trapeze bars, and Tele health equipment, etc. The Contractor awarded this contract will be responsible for all stored, delivered, maintenance, pickup, etc. of DME already issued, and will also be responsible for relocating any stored equipment from previous Contractor's facility to their facility within ten (10) working days from notice of award. The Contractor must meet current Joint Commission (JC) standards for Home Care. Evidence of meeting the standards shall be submitted with the offer and will comply with any amendments and future requirements published. Failure to maintain TJC standards or equivalent standards may be grounds for termination of this Contract. Network Contracting Office 4, supporting VAMC Altoona, intends to enter into sole source negotiations with Columbia Ancillary Services, Inc. for this requirement citing authority defined under Federal Acquisition Regulations 6.302-1, Only One Responsible Source. The contract is expected to last six (6) months in duration with an Option to extend for up to an additional six (6) months if required. The NAICS code is 532283.
