- Statement of Work FORESITE SS WHEELCHAIR SEAT SYSTEM Model # LX100 Sensor Phoenix Veterans Administration Health Care Services 650 E. Indian School Road Phoenix, AZ 85012 Statement of Work Equipment being ordered: FORESITE SS WHEELCHAIR SEAT SYSTEM Model # LX100 Sensor Transaction number: 644-280-17-00008 Description of what the equipment being requested is: REPLACEMENT: Pressure injuries are skin wounds caused by unrelieved pressure that prevents blood from reaching vulnerable parts of the body such as the spine, sacrum, coccyx, buttocks, elbows, and heels. This equipment is needed to identify areas of high pressure in patients that use manual wheelchairs, scooters, or power wheelchairs. These patients are susceptible to developing pressure injuries due to increased time sitting or reclining. Identifying areas of high pressure allows the therapist to provide the appropriate DME and recommend pressure-relieving techniques that can help prevent or heal pressure injuries. According to the Centers for Disease Control (CDC), the United States spends at least $1.2 billion annually to treat pressure sores. The benefit to the patient is to prevent debilitating pain, loss of function, and decrease in quality of life that occurs with a pressure injury. Equipment life expectancy is 7-10 years. Specifications: The requested ForeSite SS Wheelchair Seat System consists of: A flexible sensor pad embedded with thousands of sensor cells that continuously measure the patient's body surface pressures. A touch screen tablet that displays real-time images of elevated pressures. ForeSite SS software that shows various pressures applied to the sensor, highlighting higher or lower values in the pressure distribution. ForeSite SS Wheelchair Seat System Equipment life expectancy is 5-10 years depending upon use. Electromagnetic Compatibility: IEC/EN 60601-1-2:3rd Ed Description of how the equipment being requested operates: The ForeSite SS Wheelchair Seat System is used to measure body surface pressures. The sensor mat is placed on the surface of interest (for example, a wheelchair cushion). The patient is then transferred onto the sensor mat and surface. The sensors read the body pressures applied to the sensor pad. The ForeSite software displays real-time pressure images and highlights areas of high and low pressure. This allows the therapist to identify areas of high pressure which could lead to pressure injuries. Once areas of high pressure are identified, the therapist can recommend the appropriate durable medical equipment (DME) and educate the patient regarding pressure-relieving techniques. It is important to identify and address high areas of pressure because according to the Centers for Disease Control (CDC), the United States spends at least $1.2 billion annually to treat pressure sores. The benefit to the patient is to prevent debilitating pain, loss of function, and decrease in quality of life that occurs with a pressure injury. SECBOC in room 1603. Phoenix VA Bldg 34 room 1009. Who will be using the equipment being requested? Occupational Therapy staff will be using equipment for patient treatment at SECBOC. Recreation Therapy staff will be using equipment for patient treatment Phoenix VA. Veterans will be sitting on this equipment and OT staff will evaluate for pressure areas. Specific OT Staff, who will be using the equipment to perform evaluations, have been trained in using this piece of equipment. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The Solicitation is issued as a Request for Quotes (RFQ). This solicitation is being conducted under FAR Part 13 Simplified Acquisition Procedures (SAP).
