This is a notice of intent of The Louis Stokes Cleveland Department of Veterans Affairs Medical Center, 10701 East Boulevard, Cleveland, Ohio 44106 to negotiate sole source with First Nation Group dba Jordan Reses Supply Company for supplying The Department of Veterans Affairs Medical Center, Cleveland, Ohio, Prosthetic Services, with: Item Information: 200 EA Philips Respironics DSX500H11 DreamStation Auto CPAP with Humidifier 200 EA Philips Respironics 100605C Modem, DreamStation Cellular Non-Oximetry 200 EA ResMed 32707 AirSense 10 Autoset with HumidAir Supplies Includes: Philips Respironics DreamStation Auto CPAP with Humidifier - top-mounted humidifier hose, swivel and front-facing display - DreamMapper technology - Advanced event detection - Mask type resistance control - Mask fit check - Oximetry capable - Encore patient management system with optional Adherence Profiler - DreamMapper patient self-management system compatibility - Fixed, adaptive, heated tube - Pre-heat humidifier option - Modular design with 1 piece, dishwasher-safe water chamber ResMed AirSense 10 Autoset with HumidAir - advanced environment detection and adjusting - user-friendly controls, intuitive interface and color LCD screen - The AutoRampTM feature - Easy-Breathe motor - SmartStartTM function These supplies are for use at Louis Stokes Cleveland VA Medical Center. The supplies requested will be use in the Pulmonary Department at the Cleveland VAMC. The Government believes that only one business can satisfy the agency requirements and intends to negotiate with only one source under the authority of FAR 13.106(b) access to proprietary information. Other persons may identify their interest and capability to respond to this requirement by January 11, 2018 at 12:00PM EST. Such capabilities information will be used solely for the purpose, of determining, whether to conduct this procurement on a competitive basis. All information shall be furnished at no cost or obligation to the Government. A determination by the Government not to open the requirement to competition based upon responses to this notice is solely within the discretion of the Government.
