Description
This is not a solicitation announcement. This is a sources sought synopsis only. The purpose of this is to gain knowledge of potential qualified sources and their size relative to NAICS 238330, Standard Size $15.0 Million. Responses will be used by the Government to make appropriate acquisition decisions. Responses are not considered responses to any potential solicitation announcement. Scope of Work The Central Arkansas Veterans Healthcare System (CAVHS) located in Little Rock requires replacement of carpet in the Library on the 7th floor. The current carpet and flooring is very old and being held together with tape. The flooring requires the removal of the current carpeting in the 7th floor Library, and disposal of carpeting from the CAVHS campus. The contractor shall prep floor for new carpet and install new carpet. The vendor must also provide a service and maintenance agreement which will include adjusting, repairs, and replacing any malfunctioning equipment. Response to this notice should include a capability statement, and three (3) customer references to which you have provided similar services. All interested firms shall provide the following information: company name, address, point of contact, phone number, email address, capability statement, and business size pursuant to the NAICS Code 238330, Standard Size 15.0 Million. Responses to this notice shall be e-mailed to Jacqueline.Simpson3@va.gov. Telephone responses will not be accepted. Responses must be received in writing no later than Wednesday, January 24, 2018 at 10am, CST. This notice is to assist the VA in determining sources only. A solicitation is not currently available. If a solicitation is issued it will be announced at a later date, and all interested parties must respond to that solicitation announcement separately from the responses to this announcement. Responses are not a request to be added to a prospective bidders list or to receive a copy of the solicitation. STATEMENT OF WORK Carpet Installation INTRODUCTION Contractor shall provide all required labor, material and equipment, including travel cost to remove existing old rolled carpet, and replace with carpet tiles in John L. McClellan Library 7th Floor Little Rock. The below diagram shows the 3,385-sq. ft. at Central Arkansas Veterans Healthcare System (CAVHS) - John L. McClellan Library. This is brand name or equal to Tandus Centiva Texture, Map 11129, Color Pictogram 12802, Carpet Squares 24x24. Extra for attic stock. (SEE DIAGRAM BELOW) Period of Performance Vendor shall begin repairs as soon as purchase order is issued. Place of Performance CAVHS, 4300 W. 7th, Little Rock, AR SCOPE OF WORK Contractor shall schedule the install date with Sarah Smith at 501-313-9347 and alternate Mike Armack 501-257-1989. The purpose of this contract it to replace carpet in Little Rock Library 7th floor. The current carpet and flooring is very old and being held together with tape. This flooring contributes to the sanitation and aesthetics in a building used for Veteran s. Any part of service not covered by this services contract will not be installed without first procuring a formal purchase order. The contractor shall provide all labor. The contractor shall be responsible for moving furniture as required to complete the project. A Library representative will provide guidance if furniture is required to be moved to a central location within the 7th floor during the project. The contractor shall remove the current carpeting in the 7th floor library, and then remove the carpeting from the CAVHS campus for final disposal as well as prep floor for new carpet. The contractor shall install the new carpet, installation precautions shall be in compliance with VA standards. The contractor shall install the modular carpet, which shall be laid on prepped floor. The contractor shall install rubber base in designated areas. Upon completion of the job contractor shall clean-up areas to the satisfaction of the N&FS representative. c. The contractor shall obtain all necessary licenses and or permits required to perform this work. Contractor shall take all precautions necessary to protect persons and property from damage during performance of this contract. Contractor shall be responsible for any injury to himself or his/her employees, or others, as well as for any damage to personal property that occurs during the performance of this contract that is caused by him/her or his/her employees fault or negligence. The contractor shall maintain personal liability and property of damage insurance prescribed by the laws of the state of Arkansas. d. Contractor shall provide services during 8-5 Saturday and Sundays only. Install carpet, Demo carpet, Demo adhesive, minimal furniture moving, Dust barrier, Air Machine, Rubber base, base adhesive, Base Labor. Tandus Centiva texture map 11129 color pictogram 42802 carpet squares with impervious backing. Extra for attic stock. The contractor shall be responsible for removing old carpet at a location off station. No changes to the scope of this contract will be made unless approved by the contracting officer. Contractor must meet the requirements as required for an Infection Control Risk Assessment (ICRA) permit and have the permit posted in an obvious place in the workspace. Must meet safety and infection control functions and provide materials as needed to meet the ICRA and safety requirements. A. CONTRACT AWARD MEETING The contractor shall not commence performance of the services identified in this SOW until the Contracting Officer (CO) has conducted a post-award conference, or has advised the contractor that a post-award conference has been waived. B. GENERAL REQUIREMENTS The Contractor shall warrant that carpet, VTC flooring, and base installation will be done in a workman like manner and the installation will be acceptable as determined by the government. C. SCHEDULE FOR SERVICES Saturday and Sunday only from 8AM-5PM, excluding Federal holidays. The contractor shall schedule with the N&FS representative for an agreeable installation date. D. CHANGES TO THE STATEMENT OF WORK The Contracting Officer (CO) is the only individual authorized to approve changes or modify any of the requirements under this contract. The Contractor shall communicate with the POC on all matters pertaining to the work being performed. The foregoing is not to be construed as authorization to interpret or furnish advice and information to the Contractor relative to the financial or legal aspects of the contract. Enforcement of these segments is vested in and is the responsibility of the CO. Only the CO is authorized to make commitments or issue changes affecting price, quantity or performance of this contract. In the event the Contractor effects any such change at the direction of any person other than the CO, the change shall be considered to have been made without authority and no adjustment shall be made in the contract price to cover any increase in cost incurred as a result thereof. E. GOVERNMENT RESPONSIBILITIES 1. The N&FS representative shall coordinate an installation date with the contractor and provide technical guidance as required during the project. 2. Quality Assurance Surveillance Plan: A N&FS representative shall periodically monitor the contractor s performance during removal and installation activities. Unsatisfactory performance should be reported to the CO for resolution. F. CONTRACTOR EXPERIENCE AND/OR REQUIREMENTS 1. Contract employees shall be trained in performing the installation of the carpet and base listed in the scope of work. 2. Contractors shall be equipped with all necessary tools, equipment, and any Personal Protective Equipment (PPE) to perform the work safely, effectively, and timely. 3. Contract employees may be required, upon request, to provide the N&FS representative with Material Safety Data Sheets (MSDS) prior to bringing any chemicals to the facility for use. The MSDS shall be reviewed and approved by the N&FS representative prior to the chemicals being used. G. CONFIDENTIALITY AND NONDISCLOSURE 1. The Contractor understands and agrees that confidential information (patients, employee, or facility) must be secured and protected at all times. The Contractor and its personnel shall be held liable in the event a breach of confidentiality occurs as a result of the Contractor or its employees. H. CONTRACTOR PERSONNEL SECURITY REQUIREMENTS 1. Position Sensitivity: The position sensitivity/risk level has been designated as Non-sensitive /Low Risk.
