- AMENDMENT A00002 The following question has been submitted to the Contracting Officer regarding this solicitation. The question and response represent all questions submitted to the Contracting Officer prior to the deadline established in Solicitation No. 36C25618Q0090 as of 2:00 p.m. local time (CST) Friday, January 16, 2018. No more questions accepted. Bidders are instructed to submit their bids in response to the solicitation as-is. Any remaining issues will be addressed with the successful bidder. Questions and responses are provided for bidder s information and use. Vendor question 1: Under 5.0 Government Owned Equipment, Philips EZ Diagnost Eleva quantity 2, can you clarify if there are 2 Philips Diagnost Eleva units, each with a radiography tube and a fluoroscopy tube, for a total of 4 tubes? VA response A1: Yes, a total of 4 tubes. Vendor question 2: The Introduction/Background paragraph in the Scope of Work references MRI, however, I do not see MRI on the equipment list. Will there be MRI units? Please list make/models/quantities. VA response A2: Yes, there is 1 MRI unit, a Philips Ingenia 1.5T Vendor question 3: Can you please furnish the last survey dates of each piece of equipment or when the equipment needs testing next? VA response A3: August 2018 the next cycle of inspections is due. Vendor question 4: Is there a specific format you would most prefer to see in our quote? I want to insure what we provide is as reader-friendly as possible. VA response A4: Broken down by cost per device from the Statement of Work, 5.0 Government Owned Equipment, with the addition of the Philips Ingenia 1.5T MRI Room (29 device line items) Vendor question 5: Is a template that you expect all contracting companies to use? VA response A5: No, there is not a universal template.
