Description

This is a combined synopsis/solicitation for commercial items and services prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. Solicitation number 36C26218R0255 is issued as a Request for Quotation (RFQ) and constitutes the only solicitation. It is the offeror s responsibility to monitor this site for the release of amendments (if any). This notice and the incorporated provisions and clauses are those in effect through the Federal Acquisition Circular (FAC) 2005-95, effective January 19, 2017. This is a competitive solicitation; 100% set-aside for SDVOSB. The North American Industry Classification System (NAICS) code for this acquisition is 541513 Computer Facilities Management Services and the small business size standard is $27.5M. Offeror must be registered in the System Award Management (SAM) at https://www.sam.gov in order to be considered for award. See Section B.5 for Schedule of Services and D.2 for Price/Cost Schedule for the contract line item numbers (CLIN), quantity and unit of measures. Description of services: The Government is seeking a qualified contractor to provide a timely customized assessment of Veteran customer satisfaction at the point of service, immediate real-time feedback for Veteran satisfaction issues, data analysis, regular timely feedback reports, and ongoing performance improvement coaching for the VA Loma Linda Healthcare System. See Section B.4 for the Statement of Work (SOW). Period of Performance: The work to be performed is anticipated to begin April 1st, 2018 thru March 31st, 2019. The provision at Federal Acquisition Regulation (FAR) 52.212-1, Instruction to Offerors - Commercial Items (Oct 2015), applies to this acquisition. See Section E.2 for the addendum to FAR 52.212-1. A similar provision at FAR 52.212-2, Evaluation Commercial Items is used. See Section E.3 for the Basis for Award. Offeror shall include a completed a copy of FAR 52.212-3, Offeror Representations and Certifications Commercial Items, with its offer. The clause at FAR 52.212 4, Contract Terms and Conditions --Commercial Items, applies to this acquisition and full text may be accessed electronically at http://farsite.hill.af.mil. The clause at FAR 52.212 5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders -- Commercial Items, applies to this acquisition. See Section C.9. Additional contract requirements necessary for this acquisition is attached. See Section B.4 for Special Contract Requirements. Page 3 of 3 Offer is due on February 1, 2018 at 3:00 PM PST.
