Description: Past Performance Evaluation 14 Day Meal Menu Performance Work Statement Combined Synopsis and Solicitation This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in the Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation number W912TF-18-T-0005 is being issued as a Request for Quotation (RFQ) for Course Meal Preparation and drop-off at the Regional Training Site, 96 Sheep Davis Rd, Pembroke, NH. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-91, Effective 30 September 2016 and the Defense Federal Acquisition Regulations (DFARS) Change Notice 20171228. It is the contractors' responsibility to be familiar with applicable clauses and provisions. This acquisition is reserved for Total Small Business concerns. The North American Industrial Classification System Code is 722320 with a small business size standard of $7.5 million. This is a firm fixed price requirements type contract for non-personal services to provide all personnel, supervision and all other items necessary to provide meals per the attached Performance Work Statement. The requirement is for preparation and delivery of meals 3 times a day breakfast, lunch and dinner for 33 students. Day 28 (March 28, 2018 requires only breakfast). Delivery of meals will be done in IAW Performance Work Statement. Evaluation Criteria: The Government will award the contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous and best value to the Government. 1) Technical Capabilities: Offerors are required to provide a summary of experience and technical capability to support performance of the meal preparation for 33 students for breakfast, lunch and dinner, in addition to delivery of meals three times a day IAW the Performance Work Statement. 2) Past Performance: Provide three points of contact for similar work performed. Include, Name, Position, Organization, Telephone and E-mail. Please have references complete the attached past performance statement and return to Cynthia.thompson2.mil@mail.mil. 3) Price: Provide a firm fixed price for preparation and delivery of meals IAW Performance Work Statement and any other applicable costs. Technical and past performance, when combined, are equal to cost/price. All offerors must be registered in the System for Award Management (SAM) database.
