Technical Certification Form: List whether or not the item you are quoting meets the listed specifications. Complete and return with your quote. Schedule of Items. Please list your price information here.. Complete and return with your quote. RFQ document describing this requirement. For your information. The Defense Finance and Accounting Service, Indianapolis (DFAS-IN) has a requirement for 3 types of chairs, various quantities. This is a 100 percent Small Business Set Aside. This is a Brand Name or Equal requirement with preference for items quoted to be the Brand Name specified in order to match the office's existing chairs. An attachment is included, for your reference, Pictures of Current Chairs, showing the current chairs and the current fabric for Line 0001. For your quote to be considered for award you must complete and return the following three forms (see attachments): 1) Schedule of Items Table 2) Technical Certification Form 3) DFARS 252.225-7000--Buy American--Balance of Payments Program Certificate Provision If you do not complete and return these three forms your quote will not be considered for award. END
