Description
The Great Lakes Acquisition Center (GLAC) in Milwaukee, WI is searching for vendors with the capability to install and maintain a duress alarm system, located at the VA River Valley Integrated Health Center, La Crosse Community Based Outpatient Clinic (CBOC), WI. The vendor must be able to meet all the minimum requirements stated in the statement of work (listed below). Interested responsible sources must submit a summary of their capabilities to this office by 0900 CST on Monday, February 12, 2018 to include detailed description of the firm s experience in maintaining equipment, DUNS number, name and phone number of a point of contact in case further information is needed. Summaries must be submitted via email to Lori.Eastmead@va.gov. This RFI is issued solely for information and planning purposes to determine availability of qualified vendors and does not constitute a solicitation. All information received in response to this RFI that is marked as proprietary will be handled accordingly. The procurement will be completed under NACIS 561621. Please note that this is not a request for proposal and the Government is seeking information for market research purposes only. The Government may or may not issue solicitation documents. Look for any further information in FedBizOpps. Telephone inquiries will not be returned. STATEMENT OF WORK La Crosse CBOC s Duress Alarm System Scope of Work. The contractor is to furnish and install a complete stand-alone Active Shooter Duress Alarm System with maintenance at the VA River Valley Integrated Health Center La Crosse CBOC, and the VA River Valley Outpatient La Crosse CBOC. Specific Tasks Contractor is to provide all equipment needed for proper operation of the Active Shooter Alarm system along with full operational installation of the system. Contractor is to work with the COR and Local staff to setup best time to do the work that will not affect patient care. Once a time has been set, the contractor will be responsible providing all the required equipment that was quoted to have a fully operations Active Shooter Alarm System. Equipment at minimum for the VA River Valley Outpatient CBOC listed below. 1 - V21IP, 6160, BJ&C PER MAR 1 - ULTD-ZN Wireless Receiver Tran 1 - Relay Module 4 - hold Up Switch X-MTTR Tamp UL 2 - 2-TN Indr SRN 95DB 1 SMP3 WCABCAM Locktransformer 1 4G Radio F/VISTA 21IP 1 112 (Hold Up Alarm) 1 128 (BA Service) 1 144 GSMV (RadioNet) Equipment at minimum for the VA River Valley Integrated Health Center CBOC listed below. 1 V128FBPT Kit F/PER MAR Sec 1 V-Plex 8-Zn Expander 1 - ULTD-ZN Wireless Receiver Tran 1 - Relay Module 4 - hold Up Switch X-MTTR Tamp UL 2 - 2-TN Indr SRN 95DB 1 SMP3 WCABCAM Locktransformer 1 GSM/Internet Cmmrcial Fire 4G 1 112 (Hold Up Alarm) 1 128 (BA Service) 1 144 GSMV (RadioNet) The contractor is then required to fully install providing all required materials, and work to make an operations system at a time that is convenient to the La Crosse CBOC s as not to affect Patient Care. Once the system has been installed the contractor is to provide an operational test of the system in front of the COR, CBOC Liaison, or Nurse Manager of the CBOC. The contractor is will provide all agreed upon service for this system. Security Requirements: Based on the review of VA Handbook 6500.6, Appendix A, the certification and accreditation (C&A) requirements do not apply; a Security Accreditation Package is not required. Furthermore, VA sensitive information will not be at risk based on the details of the contract. Required Documentation and Reportable: 4.1 - Manufacturer s literature, hardware, access controls, and installation/operating instructions. 4.2 - Upon completion of any work on the system a detailed report of work completed and issues found. 5. Entering/Exiting the CBOC: 5.1 - During normal hours of operation, Monday through Friday, the contractor shall sign in with the CBOC Liaison or Nurse Manager in charge of the CBOC. The contractor should also let the COR of the project know when work is to be done. 5.2 - If work needs to be done other than normal hours of operation the contractor shall coordinate with the CBOC Nurse Manager and CBOC Liaison for access to the Clinic. 6. Performance Monitoring: 6.1 - Inspection/Acceptance; the Government reserves the right to inspect or test any supplies or services that have been tendered for acceptance. 6.2 - The Government may require re-performance of nonconforming services at no increase in contract price; the Government must exercise post-acceptance rights within a reasonable time after the nonconformity was discovered. 6.3 - The contractor shall take all necessary precautions to protect government property and shall repair any damage caused by their efforts. 7. Place of Performance: Work Performed for this Statement of Work will be performed partially at the VA River Valley Integrated Health Center CBOC 1525 Losey Blvd. S. La Crosse, WI 54601, with the rest of the work being performed at the VA River Valley Outpatient CBOC 2600 State Road La Crosse, WI 54601. 8. Period of Performance: Within fifteen (15) days of the award of this contract, the Contractor and the Government shall agree upon a date for install of both CBOC s alarm systems. All work affecting patients shall be coordinated in advance by the Government and the Contractor to have minimal impact on the patients. 9.
