- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. SOLICITATION #: FA4830-18-Q-Z003. This combined synopsis/solicitation is issued as a Request For Quotes This RFQ document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular (FAC) 2005-96, Defense Acquisition Circular (DPN) 20171228 and Air Force Acquisition Circular (AFAC) 2017-1003. The North American Industry Classification System is 611699. The size standard is $11,000,000.00. SCHEDULE FOR QUOTE: CLIN Description Quantity Unit of Issue Total 0001 Paramedic Refresher Training/Advance Medical Training for 15 Students, 12-14 February 2018. 1 Ea The contractor shall provide logistical support, training areas, HH60 training simulator, and instructor expertise in live tissue training. The contractor must be an accredited vendor and in compliance with applicable regulators and are APPROVED for listing on the United States Air Force (USAF) Approved Vendor List (AVL) dated 3 May 2017. Additional Detail: See attached Statement of Work. Include all discounts that may apply. Include CAGE code with quoting information. Provision at FAR 52.212-1, Instruction to Offerors Commercial Items applies to this acquisition. Request for Quotation (RFQ) should be submitted and shall contain the following information: RFQ NUMBER; TIME SPECIFIED FOR RECEIPT OF OFFERS; NAME, ADDRESS; TELEPHONE NUMBER OF OFFEROR; TERMS OF THE EXPRESSED WARRANTY; ANY DISCOUNT TERMS AND ACKNOWLEDGEMENT OF ALL SOLICITATION AMENDMENTS (if applicable). Quotes must indicate quantity, unit price and total amount for each item. Quoters shall include a statement specifying the extent of agreement with all terms, conditions and provisions included in the solicitation. Quotes shall also contain all other documentation specified herein. BASIS FOR ANY RESULTING AWARD: The Government will award a purchase order to the quoter whose price is judged to represent the best value to the government. Quotes will be evaluated on lowest price technically acceptable. All quoters must be System for Award Management (SAM) registered, www.sam.gov.
