Description

(i) In accordance with FAR Part 13, this combined synopsis/solicitation for commercial items is prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The Department of Veterans Affairs, Southeast Louisiana Veterans Health Care System New Orleans has the need to procure Surgical Tower. (ii) The combined synopsis solicitation number is 36C25618Q0210. It s issued as a Request for Quotation (RFQ). (iii) This combined synopsis solicitation RFQ and the provisions and clauses incorporated are those in effect through Federal Acquisition Circular (FAC) 2005-95 (01-19-2017). (iv) The combined synopsis solicitation is unrestricted. The NAICS is 339112. Business size standard is 1000 employees. (v) This combined synopsis solicitation is for the purchase of a complete Laparoscopic and Endoscopic Surgical Solution (Surgical Towers). Responses shall be on an all or none bases, no partial submission. The Government reserves the right to make no award at all. See Attachment A: Price/Cost Schedule (vi) A description of the items to be acquired, and/or applicable Government need(s) as follows: The contractor shall provide all resources necessary to accomplish the deliverables described in the statement of objectives (SOO), except as may otherwise be specified. See Attachment: B Statement of Objectives (vii) The equipment shall be delivered including system set up and initial training within thirty days (30) after award at the expense of the offeror, FOB destination. See Attachment: C Contract Clauses and Provisions (xiv) Quotes are due on/or before March 7, 2018 at 3:00 PM CST by e-mail only at michelle.bocage@va.gov. Offers received after the exact time specified in the solicitation are considered late and may be considered if the action would not unduly delay the acquisition or are deemed to be in the best interest of the Government. Note: Vendor Questions: All questions regarding this RFQ shall be submitted in writing and sent via email to michelle.bocage@va.gov no later than February 21, 2018 by 3:00PM CST. Answers will be posted no later than three business days by 3:00PM CST February 27, 2018 to FBO. Site Visit Surgical Towers- A one-time site visit will be conducted February 15, 2018 beginning at 9:00 am CST, 2400 Canal Street, New Orleans, LA 70120. The site visit will begin promptly at 9:00 am followed by a tour of the project site. Please respond to michelle.bocage@va.gov no later than February 9, 2018 2:00 PM CST with the name and email address of the person attending the site visit. All questions from the site visit shall be submitted in writing to michelle.bocage@va.gov.
