Description
SUBJECT: TAI CHI SERVICES THIS IS NOT A REQUEST FOR PROPOSALS (RFP) OR A REQUEST FOR QUOTATIONS (RFQ); IT IS STRICTLY A REQUEST FOR INFORMATION (RFI), AND A CAPABILITIES STATEMENT. NEITHER UNSOLICITED PROPOSALS NOR ANY OTHER KINDS OF OFFERS WILL BE CONSIDERED IN RESPONSE TO THIS RFI. NO CONTRACT WILL BE AWARDED PURSUANT TO THIS ANNOUNCEMENT. DISCLAIMER: This RFI is issued solely for information and planning purposes only and does not constitute a solicitation. All information received in response to this RFI that is marked as proprietary will be handled accordingly. In accordance with FAR 15.201(e), responses to this notice are not offers and cannot be accepted by the Government to form a binding contract. Responders are solely responsible for all expenses associated with responding to this RFI. SUBJECT: The Department of Veterans Affairs is conducting market research in order to provide a Tai Chi instructor to the Martinsburg, WV Veterans Affairs Medical Center. The government anticipates a awarding a one Firm Fixed Price contract that will provide the services listed within the below attachment. OBJECTIVE: See attachment. RESPONSE INSTRUCTIONS: Name and Address of Company Any small business designation as certified by the Small Business Administration A through description of experience in managing support requirements of this nature. CONTACT INFORMATION: Any questions related to this RFI shall be directed to Caitlin Hippensteel at caitlin.hippensteel@va.gov. All information regarding Capabilities Statements or any other proprietary information relative to this RFI shall be submitted via email to caitlin.hippensteel@va.gov no later than 2:00 pm EST on 2/20/2018. Attachment 1: Description of Work: 1.1. The contractor will provide one (1) hour of instruction on Tai-Chi every Wednesday for 52 weeks (52 classes) beginning February 5, 2018 at the Martinsburg VA Medical Center, Martinsburg West Virginia. 1.2. The goals include but are not limited to giving students tools necessary to bring themselves into a state more conducive to the goal of attaining optimum mental and physical health. At the start of the program, each Veteran is asked to participate in a Tai Chi survey which rates their current levels of pain, stress and fatigue. Participants are also asked how well they are able to sleep. After completing the Tai Chi survey Veterans begin learning the steps of Tai-Chi. Once they have reached the 16th session, the participants complete the same Tai Chi survey again. 1.3. The Contractor will coordinate with the Supervisor of Recreation Therapy to ensure all classes meet appropriate demands. A list containing the names of Recreation Therapy representatives will be provided to the contractor upon awarding of this contract. 1.4. Tai Chi classes will begin within one (1) week of the contractor receiving the Notice to Proceed (NTP). 1.5. Coordination with the VA: The contractor shall coordinate with the Supervisor of Recreation Therapy to establish and maintain a current master schedule and a detailed class roster. The Contractor will verify attendance by checking identification with driver s license or VA ID against class roster. Contractor will document class attendance and will submit rosters weekly to the Supervisor of Recreation Therapy. The Contractor will abide by Department of Veterans Affairs HIPAA and PII policies and procedures. 2. Description of Supplies: 2.1. The Tai Chi therapy sessions will be held in a well cooled and lit space conducive to a wellness environment. All necessary equipment will be provided by the Contractor. The government will provide mats and towels for all participants. 2.2. The Contractor will provide any instructional material and visual aids necessary to successfully provide the therapy. 2.3. The Contractor will coordinate any weekly schedule changes, within 48 hours, with the Supervisor of Recreation Therapy.
