Description

SF 18 Complete and return The US Department of Agriculture Agricultural Research Service in Albany Ca has a requirement to dispose of hazardous waste in compliance with Federal and State requirements. Attached is an SF18 form to be completed and returned to the attention of Fred Gifford as listed in this notice. The price schedule represents a quantity of one each and the actual quantity to be picked up quarterly may increase or decrease depending on usage. Proposals will be based on the prices presented for one each and the total price for the base year is to put on the SF18 form. Also attached and is to be completed is the complete breakdown of prices. If an item will be provided but there will not be a cost associated just put $0.00 on the breakdown sheet. For option year pricing the CPI index will be used when it is time to exercise the option. Only put in pricing for the line item O1 at this time. Evaluation will be based on prices presented and successful past performance. NO SET-ASIDE UNRESTRICTED
