THIS IS A SOURCES SOUGHT ANNOUNCEMENT ONLY. This is not a solicitation for proposals, proposal abstracts, bids, or quotations. The purpose of this notice is to obtain information regarding the availability and capability of all qualified sources to perform a potential requirement. Responses to the information requested will assist the Government in determining the appropriate acquisition method. The Great Lakes Acquisition Center is planning to solicit for a vendor to provide the following service for VISN 12: Contractor will provide the services for the Tomah Veterans Administration (VA) Medical Center, 500 East Veteran Street, Tomah, Wisconsin. The Tomah VA Medical Center has an occasional need of Court Reporter services for Administrative Investigative Boards (AIB), Arbitrations and potentially for other third party appeals. The Court Reporter will provide necessary records required which are used for settlements or decisions that are binding and legal at the Tomah Veterans Administration (VA) Medical Center, Court Reporter services may be used but not be limited to, the following: Record and provide a record of testimony for Administrative Investigation Boards. Record and provide a record of testimony for Arbitrations or other third party actions as deemed appropriate by the Medical Center Director. For other instances the Medical Center Director deems necessary. Will demonstrate excellent, polite, team-building communication skills with veterans and staff. Provide a finished type written document, double spaced, pages and individual lines numbered. Provide an electronic copy upon request. The Great Lakes Acquisition Center/ VA Milwaukee is conducting a market survey to determine the existence of potential small business concerns (e.g., HUBZone, 8(a), SDVOSB, Woman Owned, etc.) with the capability to accomplish this Service Interested parties shall provide a brief overview of their firm s capabilities and past experience with services similar in scope to this service. All information shall be submitted in Word or PDF format, not to exceed 10 pages, including all attachments. Each submittal shall also include the following business information: DUNS number Organization Name Organization Address Point of Contact (including name, title, addresses, telephone number, fax number and Email address) Business size Socio-economic status ( IF SDVOSB or VOSB provide SDVOSB or VOSB CVE verification) Firms shall identify whether or not they are a small business or other than small business in accordance with the size standard for NAICS code 541199. If another NAICS code may be applicable, please identify the NAICS in your capability statement. All information must be received no later than: February 19,2018, 2:00 PM, CST.
