- This is a solicitation for a Firm Fixed-Price for a Web-based Scheduling Package/Service Software in accordance with the format in the Federal Acquisition Regulation (FAR) Subpart 12.6, Streamlined Procedures for Evaluation and Solicitation for Commercial Items, as supplemented with additional information included in this notice. Northern Navajo Medical Center is requesting proposals from this solicitation. A written solicitation will not be issued. This a Small Business Set-Aside. Salient Characteristics: - Service must offer 24/7 personal support - Service must allow easy access to users of their schedules, trade schedules with co-workers and/or request shifts from on centralized online location. - Service must allow managers to approve/disapprove requests for schedules or to trade schedules. - Service must Include mobile access/Import of schedules so Administrative and end-users may access their schedules from any internet device. - Service must provide ability to print a specific schedule or individual schedule for providers. - Schedule Administrators must have the ability to color code schedules, schedule shifts & physicians based on their preferences. - Schedule Administrators must have the ability to override rules of automated scheduling system. - Each user must be able to customize their own schedule view by date range and must be able to quickly and easily filter or otherwise be able to look at only their schedules or the schedules of everyone in their department. - The scheduling administrator must be able to control who can access the schedules and what each person allowed to view. - Service must provide ability for end user to request vacation or swap shifts, which are then finalized by the Administrator. Approval notifications automatically sent immediately by e-mail. - Service must be capable of automatically scheduling shifts and must be capable of recording a history of every staff member's shifts including historical holiday assignments. Service must be capable of determining shift equity among staff members. - Service must be capable of providing reports showing current and past schedules statistical reports tailored by Administrative Users and personalized reports for end users. - HIPAA compliant - Transaction log of changes, requests, switches, and schedule updates - Real Time notification - Tracks shift equity - Base Year and two-option year availability - Upgrade availability, if any NOTE: Please include in your quote the cost for the base and two option years. Upon implementation or if software is available, please indicate the subscription is on a monthly or annual basis. If the software requested is not readily available, it will have to be data customized and implemented. Requesting for training and go-live service. Training to be an on-site or if web-based training is available. Please indicate which one is available. Please include the following information with your quote: a) Warranty Provisions b) Delivery time after Receipt of Order c) Terms for Payment d) Contact Person, email address and phone number e) Brochures of software and service quoted f) Provide FSS Contract Number The government shall award a contract resulting from the solicitation to the responsible offeror whose offer conforms to the solicitation and most advantageous to the Government, evaluation factors and price will be considered. The evaluation factors are as follows: 1. Past Performance (within the past 3 years) - attach documents. 2. Meets the Salient Characteristics 3. Warranty Provision - attach documents The Evaluation Factors and significant sub factors when combined are significantly more important than cost or price. In addition to price, when determining best value. Consideration for an award, the vendor has to comply with all the requirements stated in the solicitation. If documents are missing, you will not be considered for an award. Software and license must meet the following salient characteristics: The North American Industry Classification System (NAICS) is 334614 - Software and other Prerecorded Compact Disc, Tape and Record Reproducing). The Government intends to evaluate proposals based on the following evaluation factors: meets salient characteristics; past performance (within the last 3 years) of similar items provided to Federal Government Agencies); and Delivery Time of software/license to facility. The government will award a contract resulting from the solicitation to the responsible offeror whose offer is conforming to the solicitation will be most advantageous to the Government, evaluation factors, best value and price considered. The Government intends to award one contract resulting of this solicitation. An award will be based on the requirement and evaluation factors. If there is a tie, the tie breaker will be the price. To be considered for an award the documents that are relative to the evaluation factor shall be included as an attachment with the submission of the quote. SUBMISSION OF OFFERS: Submit signed and date offers to the email specified in this solicitation at or before the exact time specified in this solicitation. Submit proposals on a letterhead stationary or company's quotation form as specified in this solicitation via email to Linda Conn, linda.conn@ihs.gov, (505) 368-6089. The following information shall be included in the proposal: (1) The solicitation number; (2) Name, address, email and telephone number of the offeror; (3) Technical description of the items offered in sufficient detail to evaluate compliance with requirements in the solicitation. This shall include the company literature or brochure or other documents related to the items; (4) Company EIN and DUNS number; (5) Terms for Payment; (6) Delivery Time ARO; (7) Warranty Provision; (8) Evaluation Factors - submit documents for each evaluation factor; Offer shall be submitted to Linda Conn, Contract Specialist via email at linda.conn@ihs.gov.
