- The Department of Veteran Affairs, Network Contracting Office 2 (NCO 2), intends to issue solicitation no. 36C24218R0094, for Home Oxygen Services, for multiple VA locations within the VISN 2 Southern geographic area. The following VA VISN 2 locations, their associated outpatient clinics, and their entire patient care catchment areas, will be included in the resultant contract: New York VA Medical Center, Manhattan 423 East 23rd Street New York, NY 10010 New York VA Medical Center, Brooklyn 800 Poly Place Brooklyn, NY 11209 New York VA Medical Center, St. Albans 179-00 Linden Blvd Jamaica, NY 11425 Hudson Valley VA Medical Center, Castle Point 41 Castle Point Road Wappingers Falls, NY 12590 Hudson Valley VA Medical Center, Montrose 2094 Albany Post Road Montrose, NY 10548 New Jersey VA Medical Center, East Orange 385 Tremont Avenue East Orange, NJ 07018 New Jerey VA Medical Center, Lyons 151 Knollcraft Road Wappingers Falls, NY 12590 Bronx VA Medical Center 130 West Kingsbridge Road Bronx, NY 10468 Northport VA Medical Center 79 Middleville Road Northport, NY 11768 In accordance with the Performance Work Statement, the Contractor shall furnish all supplies, materials, equipment, and personnel necessary for patients requiring home oxygen services. Contractor shall provide Registered Respiratory Therapists to train, educate, and monitor patients, as well as complete assessments of the beneficiary s home and environment. Registered Respiratory Therapists shall be licensed in accordance with the state in which they provide service. VISN 2 has beneficiaries that reside primarily in New York State and New Jersey. Contractor must be accredited by Joint Commission (JC) or another national accreditation body that meets or exceeds Joint Commission standards, and must maintain accreditation throughout the duration of the contract. There are approximately 1,500 active patients at this time. The services associated with this contract include: All logistical/administrative functions involved with the day to day operation of providing the service to the Veteran, including in home respiratory therapy visits. This includes coordinating and managing patient care, deliveries and set-ups, briefing patient on proper use of equipment, documenting use of supplies and inspection of home in accordance with JC standards concerning patient use/safety of home oxygen. The resultant contract will be a single award, firm fixed price (FFP) requirements type, for a base period of one (1) year, plus five (5) option periods. The solicitation will be issued at the Federal Business Opportunities (FBO) website (https://www.fbo.gov) on/about September 8, 2017. The North American Industry Classification System (NAICS) Code is 621610 Home Health Care Service, Small Business Size Standard $15.0 Million. Contract award will be based upon the tradeoff process in accordance with Federal Acquisition Regulation (FAR) Subpart 15.101-1. This procurement is unrestricted. In order to be considered for award, prospective Offerors are reminded that in accordance with FAR 4.11 they shall be registered in the System for Award Management (SAM) database and shall complete electronic annual representations and certifications in SAM prior to award of a contract. SAM is located at http://www.sam.gov and consolidated the capabilities of CCR/FedReg, ORCA, and EPLS. There is NO cost to use SAM. This notice is for informational purposes only and is not a request for submission of proposals. No specific response to this pre-solicitation notice is required at this time. No potential offeror list is maintained. All potential offerors should register with Federal Business Opportunities if interested in this acquisition.
