Description

The Department of Veterans Affairs, Network Contracting Office 10, Veterans Health Administration intends to solicit offers for Histology Equipment (Leica/Sakura) service and support for the VA Medical Center located at 420 N. James Road, Columbus, OH 43219. Contract will be for the period April 1, 2018 through March 31, 2019 with four (4) option years in strict accordance with terms, conditions and provisions of the contract solicitation, NAICS Code 811219. Contractors that can provide this requirement and meets the requirements below are requested to send their 1. Company Name, 2. DUNS number, 3. Business size under NAICS 811219 and 4. If the services are available on a FSS contract or not and the FSS contract number if it is available on contract, 5. Notification if they are a VETBIZ registered VOSB/SDVOSB or not. Contractors must be registered and certified in VETBIZ to be considered VOSB/SDVOSB. This is not a solicitation. This is a Request for Information, Sources Sought. Histology Equipment Service Contract Statement of Work 1.0 GENERAL Contractor shall provide personnel to complete the task of providing full service to the Histology equipment located at the Chalmers P. Wylie Ambulatory Care Center (VA ACC) Columbus, Ohio location. 1.1 CONTRACTING OFFICER S TECHNICAL REPRESENTATIVE (COTR) The Contracting Officer s Representative (COR) will be the principal liaison for the Contractor during the duration of the contract and must be contacted prior any services performed on the equipment. 2.0 SPECIFIC TASKS Provide both service and support for the following equipment for the period of 04/1/18 03/31/19 with four (4) option years. equipment Service Serial number Cryostat PM only 1333 Cryostat PM only 047743909 Microtome PM only 14291561 Microtome PM only 7161/10.2012 Processor Total service 60300214-0609 Processor Total service 60300425-0710 Prisma System Total service 61310220-0509 Coverslipper Total service 47400791 Provide all repairs and calibrations to the instruments under total service contracts. Provide all parts and labor (and return shipping costs if needed) associated with the repair and/or calibration of these instrument as incurred during this contract under total service agreements. Provide all parts and labor associated with Planned Maintenance of all instruments. The Certification and Accreditation (C&A) requirements do not apply, and a Security Accreditation Package is not required. SUPPORTING INFORMATION Service Employee Qualifications: Service Employees shall be competent to perform the specific maintenance and repairs. Hours of Work: Contractor employees will work during normal business hours, 8:00am- 5:00pm, M-F excluding Federal Holidays. Location of Work: All work will be performed at the VA ACC, 420 N. James Road; Columbus, Ohio 43219.
