- Additional Contract Clauses Statement of Work Amended 02/19/2018 PLEASE READ NOTICE IN ITS ENTIRETY TO ENSURE SUBMISSION OF A RESPONSIVE PROPOSAL!! (i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) The solicitation number is 693JF718Q000011 and is issued as a Request for Quotation (RFQ). (iii) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-97 / 01-24-2018. (iv) This RFQ is 100% set-side for small business. The NAICS code for the effort is 712110 with a small business size standard of $27.5M. To be eligible, a small business concern must be considered a small business IAW the NAICS code identified above both at the time of its initial offer and at the time of award. (v) This RFQ is issued on an "all or none" basis. These services shall include a condition report and conservation plan for the models. In consultation with MARAD, the contractor shall evaluate the models' condition and determine the level and type of repair and conservation required. See attached Statement of Work (SOW) for additional information. (vii) The period of performance will be six months from the date of award. Phase One must be completed before Phases Two or Three can begin. (viii) Provision 52.212-1, Instructions to Offerors-Commercial, applies to this acquisition. Quotes submitted in response to this RFQ must include the following in addition to the firm-fixed price required at (v) above: (a) Quote Submission Instructions 1. Demonstration of Technical Capability which documents the offeror's understanding of the requirements specific to this SOW and provides a general approach to how the offeror proposes to fulfill the requirements of the SOW. This should include, at a minimum, examples of prior projects of same or similar scope and complexity of this SOW, creation of a project and deliverable timeline for this effort, including major project milestones and estimated level of effort and confirmation of access to necessary material and facilities for the completion of contract deliverables. 2. Past Performance - Provide at least three (3) references from past work performed/managed by the company which demonstrates work accomplished within the past five (5) calendar years, at the same or similar level of scope and complexity as required by the SOW. The references must include a contract/order number, business/Government office name/location, description of service provided and order/contract value. It must also include current POC name, phone number and email address. The past performance proposal shall be no more than three (3) single spaced pages, 12pt, MS Office 2016 or compatible. 3. All offerors must present evidence of adequate insurance coverage as required by the SOW. (b) Pre-Quote Site Visit The ship models covered within the scope of this solicitation are available for viewing at the respective sites, MARAD's Division of Gulf Operations and Department of Transportation, MARAD Headquarters, at the contractor's expense. Photographs of the ship models may be taken during the site visit. (a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: (i) technical capability; (ii) past performance; (iii) price; Technical capability and past performance, combined, are slightly more important than price. (x) Offerors must include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications-Commercial Items, with its offer. (xi) FAR clause 52.212-4, Contract Terms and Conditions-Commercial Items, applies to this acquisition. (xii) FAR clause 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders- Commercial Items applies to this acquisition and is attached. (xiii) Additional contract requirement(s) or terms and conditions apply and are attached. (xiv) MARAD requires that when the contractor takes custody of objects for off-site conservation, that the contractor has obtained adequate insurance coverage to sufficiently cover any damage or loss. See the SOW for additional insurance and other contract requirement(s) or terms and conditions that apply.
