Bid Schedule Past Performance Questionnaire Performance Work Statement Combined Synopsis/Solicitation This is a combined synopsis/solicitation for commercial items (services) prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice and in accordance with the simplified acquisition procedures authorized in FAR Part 13. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The solicitation number is W912P718T0003. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-83. This acquisition is an unrestricted solicitation. The North American Industry Classification System (NAICS) code is 562111 for Solid Waste Collection and the business size standard is $38,500,000.00. The Government will award a Firm-Fixed-Price (FFP) contract resulting from this Request for Quotes (RFQ) to the responsible offeror whose offer conforming to the solicitation. This announcement does not obligate the Government to award a contract nor does it obligate the Government to pay for any bid/quotes preparation costs. Project Description: The contractor shall provide all personnel, equipment, supplies, facilities, transportation, tools, materials, supervision, and other items and non-personal services necessary provide refuse collection and disposal services for designated public use and administrative areas at Lake Mendocino, near Ukiah, California, as described in the attached performance work statement (PWS) and elsewhere within the solicitation. Lake Mendocino is a flood control reservoir in California, with numerous day use areas, campgrounds and lake access areas managed by the U.S. Army Corps of Engineers. Park areas at Lake Mendocino offer a wide variety of recreation facilities including boat launching ramps, campgrounds, full service marina, picnic areas, and sand swimming beaches. (See attached Performance Work Statement) Specification: The offeror must meet the qualification requirement in order to be eligible for award. Line Item Break Down: See attached Bid Schedule FOB Destination Lake Mendocino, near Ukiah, California PERIOD OF PERFORMANCE: Base Year: Date of award to March 31, 2019 Option Year One (1): April 1, 2019 to March 31, 2020 Option Year Two (2): April 1, 2020 to March 31, 2021 SUBMISSION REQUIREMENT: All interested firms shall submit their quote in accordance FAR provision 52.212.-1 and shall include the information as stated below. 1. Financial Bank letter (Financial status, Line of Credit, Number of overdrafts the company has had in past year, and the average balance of commercial account. (May be expressed: low 5 figure, or High 6 figures) 8. Offerors SHALL complete and submit the price schedule and include a price for each line item.
