- SOURCES SOUGHT NOTICE For Community Nursing Home Services for Ann Arbor VA Health Care System This is a Sources Sought Notice. This advertisement is not a solicitation for offers, nor is it a request for quotes. It is solely for information and planning purposes only and does not constitute a solicitation. A request for quotes/proposal may be issued at a later date. This Sources Sought/Market Survey Notice is being conducted by Network 10 Contracting Office, PO Box 492, Ann Arbor, MI 48106. The purpose of this Sources Sought is to conduct market research to determine if responsible sources exist and if there are any businesses capable of providing the requirements. The proposed NAICS Code is 623110, Nursing Care Facilities (Skilled Nursing Facilities) ($27.5 Million). The Ann Arbor VA Health Care System located at 2215 Fuller Rd., Ann Arbor, MI 48105 has a need for Community Nursing Home Services. Nursing home care will be furnished to ensure the total medical, nursing, and psychosocial needs of VA beneficiaries. The potential vendor must be located in the counties of: Washtenaw or Jackson The VA contemplates making multiple awards (If applicable). The government is interested in sources that can provide some or all of the required services. Interested parties are encouraged to respond with what they can provide (i.e. if you can only support a certain number of beds, please state what you can provide). Prospective offerors shall respond to this Sources Sought no later than 9:00am (Eastern), Monday, March 5, 2018. Send all replies to Michelle C. Ford at Michelle.Ford2@va.gov Responses should include: (1) Business Name, GSA contract number (if applicable) and Address (2) Point of Contact Name, Phone Number, and E-mail Address (3) DUNS and NAICS code (4) Business Size: SMALL or LARGE (5) Type of Business SERVICE DISABLED VETERAN OWNED, VETERAN OWNED, 8A, HUBZone, WOMEN-OWNED etc (if applicable). All businesses eligible to provide this service are encouraged to reply as this information may be used to determine potential set-asides for the above noted service. Note: No reimbursement will be made for any costs associated with providing information in response to this Sources Sought or any follow up information requests.
