Description
1 STATEMENT OF WORK A. GENERAL GUIDANCE TITLE OF PROJECT: NFPA Fire Protection and Life Safety course for Projects and Maintenance; On-Site Training SUMMARY OF WORK: The Contractor shall provide on-site compressed training overview of the National Fire Protection Association (NFPA) Codes pertinent to Fire Protection and Life Safety. Base line topic items of training are as follows. Additional segments may be added to facilitate the time allotment: Fire Suppression Systems Fire Alarm Systems Exit Signage Emergency Lighting Fire and Smoke Dampers Elevator Systems Trash and Linen Chutes Fire Extinguishers TJC Process Risk Assessments/ILSM NFPA 99 and101 Arrangement Building Occupancy Construction Classification Construction Barriers/Infection Control Fire and Smoke Barriers Penetrations Corridor Walls & Suites Hazardous Rooms Doors and Hardware Means of Egress Training will be kept at two days and conducted in WI for up to 25 participants. Location to be determined from a concise list to include Milwaukee, Madison, or Tomah. The Contractor shall provide all labor, equipment, materials, transportation, supervision, quality control, and management necessary to accomplish the deliverables as described in this statement of work (SOW). The target audience may include engineers, designers, fire personnel, health care administrators, building managers, safety personnel and anyone who uses or enforces NFPA 99, 72, and/or 101. TERM OF CONTRACT: Period of performance shall begin on the award date of the contract and continue until the Government determines the all deliverables described herein are complete. The estimated period of performance is within 90 days from award. REQUIREMENTS Period of Performance Technical Narrative Past Performance Price NFPA Fire Protection and Life Safety Propose three date options for the two-day course within 30 days of proposal due date 2- page, 1 sided narrative examples of trainings and references Fixed Price for Fire and Life Safety Course SUPPLIES OR SERVICES PRICES/COSTS Line items 0001 through 0003 below represent individually priced costs for training at different site location options. The Government intends to award one of three site-locations. Line Items 0004 may be exercised based on final count within 14 days of training date. Item Description 0001 VA GLAC at Milwaukee; Fixed price lump sum cost to train 30 people per the requirements in the solicitation. Fixed price cost shall include vendor travel, labor, materials, and equipment necessary to deliver. 0002 William S. Middleton, Madison; fixed price lump sum cost to train 30 people per the requirements in the solicitation. Fixed price cost shall include vendor travel, labor, materials, and equipment necessary to deliver. 0003 Tomah, WI VAMC fixed price lump sum cost to train 30 people per the requirements in the solicitation. Fixed price cost shall include vendor travel, labor, materials, and equipment necessary to deliver. 0004 Cost per each additional attendee above 30 students STATEMENT OF WORK Period of Performance: The contractor shall propose three different time periods for the two-day training course. The Government reserves the right to pick with a minimum 21 days - notice without change in costs. It is the Government s intention to complete training within one hundred twenty (120) calendar days or less from the date of the notice to proceed/contract award. The course shall be performed between 9:00 AM and 4:30 PM for day one and 8:00 AM and 3:30 PM on day two. The course cannot take place during the weekends, Federal Holidays, or during hours outside of the specified. Veterans Street, Tomah, WI 54660 Points of Contact The Contracting Officer is the only person authorized to approve changes or modify any of the requirements under this contract. Only the Contracting Officer is authorized to issue changes that will affect price, quantity, or scope of performance of this contract. In the event the Contractor acts on any such change at the direction of any person other than the Contracting Officer, the change shall be considered to have been made without authority and no adjustment will be made in the contract price to cover any increase in cost incurred thereof. The Government Point of Contact may provide guidance on all matters that do not affect quantity, price, or overall scope of work. For instance, minor changes in the material may be agreed upon by the contractor and POC however, doubling the amount of material resulting in additional costs must be pre-approved by the Contracting Officer. Contracting Officer: Lori Eastmead Government Point of Contact: Marvin Schaitel B. CONTRACT AWARD MEETING Post Award Meetings: The following represent minimum requirements. The contractor may suggest a different schedule. The contractor is required, at a minimum, to coordinate (1) one post-award meeting that may be done via conference call. The contractor will propose an agenda for items to cover in preparation of the training. The contractor is to schedule, at a minimum, (1) one meeting within 72 hours prior to the beginning of the training. The purpose of the meeting shall be to report all final details of the training. This meeting may be done via conference call. C. GENERAL REQUIREMENTS The VA has adopted the National Fire Codes (NFC) published by the National Fire Protection Association (NFPA), which establishes a minimum acceptable level of life safety and property protection as explained in the Office of Safety, Health, and Environmental Compliance guide at http://www.cfm.va.gov/til/dManual/dmfpfire.pdf. D. SPECIFIC MANDATORY TASKS AND ASSOCIATED DELIVERABLES Contractor shall submit their proposed approach of accomplishing the below deliverables. The contractor is free to propose a different schedule if all minimum requirements are met. Post-award, all deliverables shall be worked out between the Government POC and the contractor. DELIVERABLES Description Minimum Requirement 1 Work Plan/course schedule/agenda for Fire and Life Safety (up to 30 students per class) within 15 days of contract award 2 Deliver Student Course Material to class location Date of training 3 Conduct NFPA 99 and 72 Compressed overview Class Propose 3 dates min 30 days from award 4 Completion and NFPA evaluation CEU certificate issued to attendees within 7 days of training day Contractor Furnished Items ITEM # QUANTITY DESCRIPTION Contractor(s) with expertise 1 Contractor with expertise in delivering training on the administrative, technical, cultural, and behavioral aspects of safety. The course should be customized to VA requirements as specified. Equipment Contractor Discretion All equipment, other than what is specifically listed under GFP below, which is necessary to provide quality training shall be provided by the contractor. Student materials kits 1 per VAMC enrollee (305) Each attendee shall receive a student material set consisting of a course workbook Certification of Completion 1 per enrollee Each enrolled participant shall receive course certificate of completion on meeting instructor requisites of training. Course Lengths: The total amount of instruction time for two (2) days of instruction at seven (7) hours of instruction per day. VA Furnished Accommodations MATERIALS/SERVICES QUANTITY WHEN, WHERE, HOW TO BE FURNISHED Facility and Room with chairs and tables Accommodate up to 30 or more Based on the site options A/V Equipment/Support 1 Room Projector, Screen or 70 in TV and all hookups, Microphone, Podium Computer and internet will not be available.
