Description

This is a combined synopsis/solicitation for commercial items, prepared in accordance with the format in Federal Acquisition Regulation (FAR 12.6) as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; offers are being requested and a written solicitation will not be issued. Solicitation number 36C25618Q0364 is issued as a Request for Quote (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-84. The overall purpose is to provide Research Lab Equipment for the Southeast Louisiana Veterans Health Care System (SLVHCS), 2400 Canal St, New Orleans, LA 70119. All work shall be completed in accordance with the Statement of Work (SOW) entitled, Research Lab 22 Equipment dated 11/16/2017. The Statement of Work within this solicitation is a non-negotiable document. Offers/quotes shall not include edits, revisions, additions, deletions, alterations, or track change comments to the Statement of Work. The associated North American Industry Classification (NAICS Code) for this requirement is 332618. This solicitation is set aside 100 % for Small Businesses (Total SB set aside). and shall not be considered for a contract award resulting from this solicitation. Offerors shall provide business name, doing business as (DBA) name, mailing address, phone number, DUNS number, CAGE code, and proof of active SAM (System for Award Management) registration when submitting offers in response to this solicitation. When preparing offers in response to this solicitation offerors shall refer to FAR 52.212-1 Instructions to Offerors-Commercial Items which provides specific guidance and instructions for offer submission. Offerors shall also familiarize themselves with 52.212-2 Evaluation-Commercial Items which provides the criteria for how offers will be evaluated.
