In accordance with FAR 15.201(e), responses to this notice are not offers and cannot be accepted by the Government to form a binding contract. Responders are solely responsible for all expenses associated with responding to this RFI. 1. In accordance with Federal Acquisition Regulation (FAR) Part 10 Market Research and Public Law 109-461, the Department of Veterans Affairs is conducting market research to ascertain if there are any companies that are capable and willing to provide artificial prosthetic limbs and services to facilities located below. This announcement is for informational and planning purposes only and is not to be construed as a commitment by the government, implied or otherwise, to issue a solicitation or award a contract. 2. The Department of Veterans Affairs (VA) has a requirement for providing artificial prosthetic limbs and services to the VA facilities within the Central Region. The contractor would be required to provide all personnel, equipment, tools, materials, supervision, facilities, transportation, (possibly training VA staff) and other items and services necessary to perform this requirement. 3. SERVICES: The Department of Veterans Affairs (VA) is seeking potential sources capable of providing fabrication of prosthetic limbs in strict conformance to the prosthetic prescription that has been provided by the VA Amputee Clinic Team. Products that are not commercial items are considered experimental and purchase of experimental products shall not be authorized under this contract. For fabrication and repair of artificial limbs, the prices proposed shall not exceed the established L-Code prices applicable to the geographic area subject to this solicitation and at the time of performance. It should be noted that repair rates for 15-minute increments are now part of the L-Code pricing schedule. The Contractor shall have a full-time American Board for Certification in Orthotics, Prosthetics & Pedorthics (ABC) certified, or Board of Certification/Accreditation International (BOC) certified prosthetist(s) on staff in good standing at each of the proposed service locations throughout the contract period. Good standing is defined as participating and achieving appropriate continuing education credits and paying dues as determined by Certifying Agency such as ABC and BOC. The Contractor shall provide semi-annual service to each veteran for whom the Contractor is managing prosthetic limb care. For the purposes of this contract, semi-annual service is defined as a follow up visit for inspection of the prosthetic limb(s) and evaluation for fit and function for all VA-approved prosthetic limb(s) the patient owns and is actively using. Typically, semi-annual service includes inspection, testing, cleaning, and checking of the prosthetic limb(s) and/or residual limb(s) to include maintenance specific to the warranty of devices. Semi-annual service shall not be incumbent upon the number of prosthetic limbs a patient owns. Two follow up visits per patient are required annually for a patient to whom the Contractor delivered a prosthetic limb(s) under this contract. Anticipated contract to be a base 12-month performance period with four (4) one-year options to be exercised at the discretion of the Government. 5. The purpose of this notice is to determine interest and capability of potential qualified companies relative to the North American Industry Classification code (NAICS) 339113, Surgical Appliance and Supplies Manufacturing with a size standard of 750 employees and The Small Business Administration (SBA) has issued a non-manufacturing waiver for NAICS code 339113. To make an appropriate acquisition decision for this requirement, the Government will use the response to this RFI. The type of solicitation to be issued and the manner of advertisement will also depend on the response to this RFI. To be considered a SDVOSB/VOSB concern, offerors shall be registered and verified in www.vip.vetbiz.gov. 6. Responses to this RFI shall be sent via emailed to Christopher.wynn@va.gov not later than 27-March-2018 CST,. Please include : VA251-18-AP-2514, Artificial Prosthetic Limbs RFI Response in the subject line of all correspondence. Telephone inquiries will not be accepted. Questions received after the specified date and time will not be considered. Responses should include the following information: Name, and address; Business size; DUNS Number; Identification of any socioeconomic categories (i.e. SDVOSB, VOSB, etc.); Please include your American Board for Certification in Orthotics, Prosthetics & Pedorthics (ABC) or Board of Certification/Accreditation International (BOC) certification. A brief capability statement demonstrating past performance with relevant contract services in Prime Contractor role. Capability Statement shall not exceed more than 3 pages. Submit Capability Statements to clearly describe their capabilities to include management experience, equipment and personnel or the ability to obtain them. Furthermore, interested sources must also meet all requirements of Federal, State, and Local Municipal codes regarding operations of this type service. Please provide detailed description if your company can provide for the entire VISN(s) specified above; Please describe your supply chain process. Does your company hold a current FSS contract in supplying artificial prosthetic limbs? If so, please state the contract number and item identification number under your FSS contract.
