- Performance Work Statment Amendment 0002 Vendors who have previously provided a proposal may resubmit a new proposal no later than 3 April 2018 at 2pm CST. Vendors must complete both Amendment 1 (SF30) and Amendment 2 (SF30) when submitting proposal. If a vendor has previously submitted a response (proposal) to our office and does not want to provide a new proposal, they must complete Amendment 1 and Amendment 2 and submit to both amendments to Jack Caniglia, jack.caniglia.3@us.af.mil and Timothy Hull, timothy.hull.6@us.af.mil. For new proposals Amendment 1 and Amendment 2 need to be part of the proposal that is submitted by the new deadline. Additional Information is being provided in response to previously asked questions; the answers to those questions are as follows: Question: Please provide the number floors in each building where there are vents to be cleaned and the number and model of dryers on each floor. for example single dryers, stack dryers, combo washer/dryer. Answer: The attached Amendment 0002 includes a revised PWS Appendix A that includes a column that identifies the floor the machines are on. Question: Please provide the distance from the back of each dryer to the outside exhaust vent. Please provide the number of turns in the vent system inside the walls or celling that run from the back of each dryer to the outside exhaust vent. Answer: With respect to FSH washer/dryer locations, they are all located on the end the facilities adjacent to exterior walls. Contractor should be equipped to clean ducts and ducting systems without complication. The dryer exhaust directly to the outside of the buildings from the floors where they are located. Question: Please provide the number of booster fans located in the dryer vent system in each floor. Please provide which dryer vents empty the dryer exhaust on the roof of the building. Answer: This question is Not Applicable due to the fact the systems are not vented out to roof tops and therefore do not require a fan system to assist in pulling the dryer exhaust up and out of the facilities. Question: May the new contractor receive the daily repair service calls via e-mail or fax? Answer: Para. 1.1.9 of the PWS has been revised to clarify; the contractor will receive Service Request tickets via email from the COR, Monday thru Friday. These service tickets will not be required to be physically picked up when they occur.
