- STATEMENT OF WORK Transaction#: 537-18-2-6072-0049 Scope: The contractor shall provide all labor, travel, supervision, materials and necessary equipment to install Luxury Vinyl Tile (LVT) on the 9th floor of the Damen Pavilion of the Jesse Brown VA Medical Center in Chicago, IL. The contractor shall be responsible for the removal and recycling of existing carpeting. The contractor shall also be responsible for the preparation and installation of rubber base, ADA compliant transitions, and LVT. One room shall be completed in its entirety before the contractor starts to work on the next room. All work shall be performed according to the manufacturer s specifications and industry standards. All work shall be completed no later than 60 days after contract award. QUALITY ASSURANCE: Installer Qualifications: A company specializing in flooring installations with minimum three (3) years experience and employs experienced flooring installers who have retained, and currently hold, an INSTALL Certification, or a certification from a comparable certification program. 1. Installers shall be certified by INSTALL or a comparable certification program with the following minimum criteria: a. US Department of Labor approved four (4) year apprenticeship program, 160 hours a year. b. Career long training. c. Manufacturer endorsed training. d. Fundamental journeyman skills certification. Specific Tasks/Deliverables. Install Luxury Vinyl Tile: Task 1: Moisture test of concrete slab. Contractor shall conduct a moisture vapor emission rate using calcium chloride (ASTM F 1869) and/or percent internal relative humidity (ASTM F 2170) tests shall be conducted in strict accordance with the most current edition of the ASTM test method. Allowable maximum moisture limits for the installation of the LVT must meet manufacturer s requirements. Task 2: Removal of Existing Carpet Tiles. Contractor shall remove the existing flooring. Task 3: Storage of Existing Flooring. Contractor shall remove and store of all existing carpet tiles for recycling. Contractor shall store the flooring in a container provided by the contractor. Task 4: Floor Preparation. Contractor shall ensure subfloors/underlayments are dry, clean and smooth. The floor shall be free of dust, solvents, varnish, paint, wax, oil, grease, and residual adhesives. Contractor shall smooth, level, and prepare all flooring surfaces to ensure new flooring adheres to the floor. Task 5: Installation of LVT. Contractor shall install 6 x36 luxury solid vinyl tile flooring utilizing Amtico 373 or a Base Acrylic Latex. VOC s -0 Calculated. Solvent-free, nonflammable, freeze/thaw stable and nearly odor-free. Does not support the growth of mold and mildew. Can be used to install resilient tile over porous and non-porous subfloors. Spreads easily, exhibits quick grab and has excellent resistance to moisture and plasticizer migration. Task 6: Installation of Rubber Base. Contractor shall furnish and install a Rubber 4 wall base with standard toe. Remove existing wall base. Rubber Base Installation: The contractor shall ensure that all walls are clean, smooth, flat and dry. The surface must be free of all dust, loose particles, solvents, paint, grease, oil, wax, alkali, sealing/curing compounds, old adhesive, and any other foreign material, which could affect installation. Remove existing adhesive mechanically do not use chemical adhesive removers or solvents. Fill all depressions, cracks, and other surface irregularities with a satisfactory quality patching compound. SBR Rubber Base salient characteristics: 1/8" (3.2 mm) thick by 4" (101.6 mm) height. Cove Toe Style Color 171 Sandstone 100% thermoset (TS) rubber wall base that is PVC free. Phthalates Free FloorScore ® Certified Wall base is manufactured in the USA Warranty - Limited 2-year warranty. Task 7: Contractor shall provide a receptacle for the old carpeting. The old carpeting will be removed from the VA premises and recycled by the contractor. Task 8: Contractor shall supply ADA compliant transitions where necessary. Hours of Operation. Normal hours of coverage are Monday through Friday from 8:00 am to 4:30 pm CST, excluding holidays. All service/repairs shall be performed during normal hours of coverage unless requested by the designee and approved by the CO or COR. One room shall be completed in its entirety before the contractor starts to work on the next room. Holidays include: New Years' Day Labor Day Martin Luther King Day Columbus Day Presidents' Day Veterans' Day Memorial Day Thanksgiving Day Independence Day Christmas Day And any other day specifically designated as a national holiday by the President of the United States. Contractor shall warrant services against defects of workmanship under normal use for one year from date of installation. Contractor personnel shall conduct themselves in a professional manner, with respect for patient care and privacy maintained at all times.
