- The Overton Brooks VA Medical Center (OBVAMC) is seeking a contractor with proven experience in providing viable fingertip sampling and media-fill testing per USP 797 Guidelines. Instruction to Offerors Guidance: This Request for Information (RFI) is not a Request for Quote (RFQ). No solicitation is being issued now. This notice shall not be construed as a commitment by the Government to issue a solicitation or ultimately award a contract, nor does it restrict the government to an acquisition approach. Any information provided to the Government because of this Sources Sought Notice is voluntary. Responses will not be returned. No entitlement to payment or direct or indirect costs or charges by the Government will arise, because of, contractor submission of responses or the Government s use of such information. This Request for Information is for planning purposes only and is intended to identify contractors in SDVOSB/VOSB/s categories who can support this requirement. INSTRUCTIONS FOR SUBMISSION Interested contractors shall submit a response via e-mail only to Leslie Robinson, Contract Specialist leslie.robinson@va.gov. Responses are due no later than April 4, 2018 by 2:00 pm Central Daylight Time (CDT). Respondents should include as part of their submission: Indicate if you can provide all services listed in the Performance Work Statement. If/when this requirement is solicited, it will be all or nothing meaning that prospective contractors must be able to provide all services. Please do not submit a price quote as this request is for market research/informational purposes only. Performance Work Statement Fingertip Sampling and Media-Fill Sampling Scope of Work: Overton Brooks VA Medical Center (OBVAMC) is seeking a contractor with proven experience in providing viable fingertip sampling and media-fill testing per USP 797 Guidelines. The vendor will be expected to provide a complete viable fingertip sampling on at least 25 employees located at Overton Brooks VA Medical Center Pharmacy. The vendor will supply cleanroom education/training. The sampling procedure will have no less than three viable fingertip sampling per employee per USP 797 Guidelines. The vendor will also complete a media-fill test on the same 25 employees per USP 797 Guidelines. This should be completed over a period of 5 days. The vendor will provide results of viable fingertip sampling and media-fill sampling in a written report documenting test results within 2 weeks of completion of each sample. If any samples have bacterial growth the samples are to be cultured for bacterial contaminate and reported back to said facility The contractor is to provide all necessary items necessary to perform these actions. The contract is between OBVAMC, as a healthcare provider, and the contractor, for the purpose of reimbursing the contractor has a set price for clean room education/training, providing fingertip testing and media-fill testing. General Requirements for OBVAMC Hours of Service: The contractor at a minimum will provide testing from 0800 to 2200 Monday through Friday. The intent of this contract is for the contractor personnel to evaluate the hand hygiene and garbing practices of the VA personnel through glove finger tip sampling. Contractor personnel to also evaluated the VA personnel for aseptic work practices using sterile fluid bacteria culture media-fill verification. The vendor is responsible for providing to OBVAMC Pharmacy Management a written report with results of all viable fingertip sampling and media-fill test within 2 weeks of completion of said test. If any samples have bacterial growth for action levels and data evaluation this will be reported to the OBVAMC Pharmacy Management in a written report. Contractor shall provider report within 2 weeks of completion of the VA personnel testing. The vendor is responsible for providing Cleanroom education/training which will include lecture, training, materials, written examination testing of pharmacy staff regarding USP 797 compliant cleanroom procudure and general USP 800 safety, and certificate of completion. The contractor is responsible for assuring that all persons, whether they be employees, the agents sub-contractors, or anyone acting for or on behalf of the contractor, are properly licensed at all times under applicable state law and/or regulations and will be subject to privileging and or background checks by the VA. Schedule of Insurance: The contractor shall procure and maintain for the entire contract period: Workmen's Compensation and employer's general liability Insurance. Prior to contract award, the Contractor shall provide proof of insurance for the entire contract period. The parties agree that such personnel shall not be considered a VA employee for any purpose and shall be considered employees of the contractor. Price Schedule: The contract rate is an all-inclusive rate. No separate fees will be allowed for such incidental costs of malpractice insurance coverage, supplies, equipment, etc. Environment of Care: The contractor shall maintain a clean and safe environment at all times. Contracting with Parties Listed on the OIG List of Excluded Individuals/Entities: In accordance with HIPAA and the Balanced Budget Act (BBA) of 1977, the Office of Inspector General has established a list of parties and entities excluded from Federal health care programs. Specifically, the listed parties and entities may not receive Federal payments due to fraud and/or abuse of the Medicare and Medicaid programs. The list of excluded individuals/entities can be found on the OIG web site at www.hhs.gov/oig. With submittal of proposal, the contractor shall provide evidence than none of the contractor s employees are on the Excluded Parties list. The certification and accreditation requirements do not apply, and that a security accreditation package is not required. Place of Performance Contractor shall furnish services at Overton Brooks VAMC located at 510 East Stoner, Shreveport, LA 71105 beginning May 1, 2018
