- map quote doc and clauses This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Quotations are being requested. Solicitation 12-52B1-18-Q-0029 is being issued as a Request for Quotation (RFQ), to provide and tailgate spread course aggregate on the Midewin National Tall Grass Prairie, Wilmington, Illinois. The North American Industry Classification System (NAICS) code is 484220 and the size standard is $27.5mil. This is a 100% Small Business Set-Aside. Pricing and technical information will be submitted to: Roger Manning, USDA Forest Service, 811 Constitution Ave, Bedford, IN 47421. NO FAX QUOTES WILL BE ACCEPTED. E-mail quotes are acceptable at rkmanning@fs.fed.us. Service Wage Rates are applicable. Quotes must include vendor's TIN (Tax Identification Number) and DUNS number. Vendors must be registered in SAMS (www.sams.gov) Quotes must include a completed copy of (FAR 52.212-3 Offeror Representations and Certifications, Commercial Items, paragraphs (c) through (m). If the Offeror has completed the annual representations and certifications electronically submit only paragraph (b) from the provisions and a DUNS number. (FAR) 52.212-2 Evaluation, Commercial Items is applicable with the following factors used to evaluate quotes: Price, technical conformity to specifications, and past performance. Clauses indicated on a separate attachment. Quotes are due April 16, 2018 at 11:00a.m. Eastern Time.
