Description
This is a Sources Sought notice. This is NOT a solicitation for proposals, proposal abstracts, or quotations. The purpose of this notice is to obtain information regarding the availability and capability of all qualified sources to perform a potential requirement. Background: The Lexington VA Medical Center, located at 1101 Veterans Dr. Lexington, KY 40502, has a requirement for Neurological Monitoring Services as described in the attached PWS. Purpose and Objectives: Your responses to the information requested will assist the Government in determining the appropriate acquisition method. NAICS code: 621999 and sized standard is $15 million employees. NOTE: Respondent claiming SDVOSB and VOSB status must be registered and CVE verified in VetBiz Registry https://www.va.gov/osdbu/. Project requirements: The contractor shall provide complete neurological monitoring services to the Lexington VA Medical Center s Operating Suites beginning July 2018 for 3 months with four (4) one-year option periods. The contractor will include operating and maintaining equipment (including monitoring devices), supplies, and labor of on-site qualified technician to perform intraoperative neurophysiologic monitoring (IONM) services in support of both neuro and vascular surgeries performed at the Lexington VA Medical Center. This is a long-term recurring need of approximately four hours per session, twice per week, for 50 weeks per year. Majority of services will be required during the standard business hours Monday Friday between 7:00 am and 3:30 pm. There shall not be a need for this service after regular hours of operation, weekends, or holidays. Anticipated period of performance: July 2018 for 3 months with four (4) one-year option periods. Capability statement /information sought: Response to this announcement shall not to exceed 10 pages and should include the following information: (1) A tailored capability statement indicating the firm s ability to provide a product and related services; (2) The respondents DUNS number, organization name, address, point of contact, and size and type of business (e.g., SDVOSB/VOSB, etc,) pursuant to the applicable NAICS code; (3) Associated contract vehicles (i.e. GSA/FSS; SEWP); (4) Information should be provided electronically in a Microsoft Word or Adobe PDF format.
