Page 2 of 3 This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is 36C25018Q0576 and is issued as a request for quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-97. The associated NAICS code is 3391134, Surgical Apparatus and Supplies Manufacturing. The Small Business Standard is 750 employees. This is a total Service Disabled Veteran Owned Small Business set-aside. The requirement is for acquisition of (1) Situate Detection System X (Console X, Body, Room Scanner), to be delivered to the Chalmers P. Wylie Ambulatory Care Center, 420 N James Rd, Columbus OH 43219 on or before May 4, 2018. Contractors, contractor personnel, subcontractors, and subcontractor personnel shall be subject to the same Federal laws, regulations, standards, and VA Directives and Handbooks as VA and VA personnel regarding information and information system security. 6.
