- Attachment contains RFQ Cover Page, Statement of Work, Quote Schedule and FAR Clauses This is a combined synopsis/solicitation for other than commercial items prepared in accordance with FAR Part 13. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The solicitation is issued as Request for Quotes (RFQ) 246-18-Q-0027. This procurement is 100% small business set-aside under NAICS code 316210 with a standard business size of 1000 employees. Contractor shall provide a firm, fixed-price rate for compression stockings, diabetic shoes to include pedorthist services. Please review the Statement of Work and Quote Schedule. The period of performance for this requirement is approximately one year. Questions are due no later than Tuesday April 16, 2018 5:00pm CST. Questions shall not be answered after this date. The closing date for receipt of quotes is Friday April 27, 2018. The offer may be submitted along with the Quote Schedule by email to Katrina.Eaton@ihs.gov or in a sealed envelope, addressed to this office, showing the solicitation number, and your name and address, no later than 5:00 p.m. CST, on April 27, 2018. Qualified vendors shall review the Statement of Work and following information then submit applicable information. TELEPHONE INQUIRIES SHALL NOT BE ACCEPTED AND TELEPHONE CALLS SHALL NOT BE RETURNED. The technical evaluation shall be a determination based on information furnished by the vendor in the submitted proposals.
