- Purpose and Objectives: The intent of this Sources Sought Notice is to identify potential offerors capable of providing Alcon Laboratories Cataract/Eye Surgery Supplies, as listed below in the Statement of Work. These items are for the Wilkes-Barre VA Medical Center in Wilkes-Barre, PA 18711. Responses to this Sources Sought Notice should demonstrate the firm's ability, capability, and responsibility to provide the principal components of supplies listed in the attached document. Responses should include the following information: Verify that the business is under 337214 of the North American Industry Classification System (NAICS), Verification of Size Standard under the NAICS, Business name, address, Point of Contact, Federal Supply Schedule (if applicable), and if the business has verification as a Service Disabled Veteran Owned Small Business or Veteran Owned Small Business. All information is to be submitted via e-mail at Daniel.Johnson44@va.gov. Information provided will not be returned. All responses shall be in the English Language. Responses are due by 12:00 P.M. (EST) on TUESDAY, APRIL 17, 2018. No submissions will be accepted after this date and time. Questions can be submitted electronically to Daniel.Johnson44@va.gov. This is a Sources Sought Notice and submissions will be used for informational and planning purposes only. This notice does not constitute a formal Request for Quote (RFQ), nor is the government obligated to issue an RFQ. In addition, the Government does not intend to pay for any information provided under this notice. The Government is not obligated to notify respondents of the results of this survey. Statement of Work (SOW) Lebanon VA Medical Center THIS IS A BRAND NAME OR EQUAL PROCUREMENT Title of Project: Activations Dock & Lock Cabinets Scope of Need: The contractor shall provide all labor, materials, and equipment necessary to procure a total of five (5) Dock & Lock cabinets, accessories, and delivery (assembly as needed) to the locations specified below. Type of Contract: Firm Fixed Price, Brand Name or Equal (Final Offers Evaluated by the COR and SME) Required Delivery Date: Three (3) units are required no later than (NLT) 05/30/2018, to Cumberland VA Clinic and two (2) units are required NLT 08/24/2018, to Lancaster VA Clinic. General Requirement: The Lebanon VA Medical Center and associated clinics has a Brand Name or Equal to PSSI Dock & Lock cabinet (4852-L-30) for secure powered mobile storage of electronic notebooks and laptops. Currently the Berks County Clinic is utilizing the PSSI Dock & Lock cabinet (4052-L-20) successfully. The Dock & Lock mobile cabinet will allow secure and ventilated storage and charging of notebooks and laptops at different locations throughout the Lebanon VA Medical center and associated clinics. Required Specifications Brand name or Equal to PSSI Model # 4852-L-30 Unit Measurement equal to 48 W x 52 H x 25 D (Add up to 5 to height for wheels) 30 Drawer units Drawer Dimensions 13.75 W x 20.25D by 3.25 H to accommodate a variety of tablets and laptops AC Power to each drawer Cat5/6 wiring capable (wiring not needed at this time) 125V/15A/60Hz Surge Suppression Power Supply UL Approved Components Mobile Base or industrial Casters Thermostatically controlled fans/ventilation Independently locking drawers with individual key assignments Master Keys Interchangeable Drawer Module Steel construction, beige powder coated finish Multiple knockouts for cabling access Full Extension ball bearing drawer slides On-Site delivery and assembly ready to use Delivery: Actual delivery dates shall be coordinated with the designated Technical Point of Contact seven (7) days prior to any scheduled delivery, but all deliveries (5 Units) must be completed no later than AUGUST 24, 2018. DELIVERY LOCATIONS: Deliver three (3) units; NLT 05/30/2018, to Cumberland VA Clinic and two (2) units are required NLT 08/24/2018, to Lancaster VA Clinic.
