Peters VA Medical Center, 130 West Kingsbridge Road, Bronx, New York 10468 Description This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) subpart 12.6, Streamlined Procedures for Evaluation and Solicitation for Commercial Items, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; are being requested, and a written solicitation document will not be issued. This solicitation is a "request for quotations (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-94. The associated North American Industrial Classification System (NAICS) code for this procurement is 333318, with a small business size standard of 1000 employees. The Department of Veterans Affairs, St. Albans Community Living Center, ENVIRONMENTAL MANAGEMENT (137), 179th Street & Linden Boulevard, St. Albans, NY 11425 is seeking to purchase NYHHS Self-Contained Compactors and BAILER All interested companies shall provide quotation(s), for the following: Supplies Line Item Description Quantity Unit Price Total Price 0001 Self-Contained Compactor; 35 Yard Self-contained Compactor with 10 HP, 3 phase motor, ram guide system, full rear door gasket, cycon cylinders, key operated start and remote power unit or similar. For Main Building Loading Dock 1 0002 For St. Albans Main Building Loading Dock - 420HD 9100 Baler for cardboard 1 0003 Self-Contained Compactor; 35 Yard Self-contained Compactor with 10 HP, 3 phase motor, ram guide system, full rear door gasket, cycon cylinders, key operated start and remote power unit or similar. For the Textile Care Facility 1 Services a. PREPARATION Site visits to NYHHS St. Albans Campus to assess the logistics of compactor & baler replacement will be on April 18, 2018 at 11:30AM. Meeting to begin at the Textile Care Facility (Laundry) Building #173. Obtain confirming measurements. Provide options on the most beneficial positioning and ease of use of the self-contained compactor and baler to be utilized. Assess any and all electric needs, upgrades, voltage, extensions, etc. b PRE-INSTALLATION Perform necessary (if any) outdoor ground maintenance on which the new compactor unit is to be placed i.e. tracks, platform and/or rails. c. INSTALLATION Perform installations of tracks, fittings and the newly purchased units. d. POST-INSTALLATION Return site visit within 30 days to ensure all aspects of the job are complete and working properly. Service calls to repair and/or adjust operation issues with the installed compactor, tracks and or fittings, shall be at NO COST. Delivery shall be provided no later than 30 days award of contract. [FOB destination delivering Line items 1 & 2 to the Main Building Loading Docks and Line item3 to the Textile Care Facility. All offerors shall submit the following: Quote with DUNS number, letter of authorization, solicitation number, phone number, fax number, point of contact, SDVOSB VIP certification and proposed delivery date for goods. All offers shall be sent to the Contracting Officer: David.Shemtob@va.gov This is an open-market combined synopsis/solicitation for products as defined herein.    The government intends to award a purchase order as a result of this combined synopsis/solicitation that will include the terms and conditions set forth herein. To facilitate the award process, all quotes, must include a statement regarding the terms and conditions herein as follows: "The terms and conditions in the solicitation are acceptable to be included in the award document without modification, deletion, or addition." Submission shall be received not later than 04-27-2018 @12:00PM via email to david.shemtob@va.gov. All questions shall be submitted via email and questions will not be accepted after 04-19-2018 @ 12:00 PM.
