- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 - Streamlined Procedures for Evaluation and Solicitation for Commercial Items and Subpart 13.5 - Simplified Procedures for Certain Commercial Items, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. USAID intends to award a firm fixed price contract for Maternal and Child Health Essential Medicines as a result of this request for quotation (RFQ). Amendment #2 to this RFQ is hereby issued to notify interested of the submission of all materials in accordance with the revised RFQ attached hereto. The Closing Date for Submission of Proposals has been extended to April 20, 2018. All other provisions and text of the RFQ remain unchanged. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-97. Federal Supply Codes/Product Services Codes: 65 -- Medical, dental & veterinary equipment & supplies; 99 -Miscellaneous. North American Industry Classification System Code: 541990 -- All other Professional, Scientific, and Technical Services. As described by USAID Automated Directives System 312, purchase and distribution of medical supplies requires prior written approval from the Global Health Bureau's Supply Chain for Health Division of the Office of HIV/AIDS (GH/OHA/SCH) in order to provide for the quality of the pharmaceuticals. All responsible sources may submit a quotation, which shall be considered by the agency. If changes are made to the closing date and time, as well as other aspects of the RFQ, this solicitation will be amended. Any amendments to this solicitation will be issued and posted on the Federal Business Opportunities (FBO) website. Offerors are encouraged to check this website (http://www.fbo.gov) periodically. It is the responsibility of the recipient of this solicitation to ensure that the solicitation has been received from the Internet in its entirety. USAID bears no responsibility for data errors resulting from download or conversion processes. If your organization decides to submit a proposal in response to this solicitation, it must be submitted by the designated date and time indicated in Block 14 of the RFQ Cover Page (SF30). Proposals received after the closing date and time will be processed as late and handled in accordance with FAR 52.215-1. Proposals submitted in response to this solicitation shall be valid for no less than 240 days. NOTE: ONLY Electronic submissions will be accepted. No other forms of Electronic submission will be accepted. Please note that the designated initial point of entry to the Government infrastructure for proposal submission is the USAID/Washington Internet server which will determine the timeliness of submission.
