- This is a Sources Sought notice. This is NOT a solicitation for proposals, proposal abstracts, or quotations. The purpose of this notice is to obtain information regarding the availability and capability of all qualified sources (small and other than small) to perform a potential requirement. Purpose and Objectives: Your responses to the information requested will assist the Government in determining the appropriate acquisition method. NAICS code: 611430 and sized standard is $11.0M. NOTE: Respondents should be registered in the System for Award Management (https://www.sam.gov. Respondents claiming SDVOSB and VOSB status must be registered and CVE verified in VetBiz Registry www.vetbiz.gov. Project requirements: Contractor shall furnish all labor, transportation, materials and expertise necessary to provide education and training of the staff that aligns with current best practices in dementia care. The course shall meet The Joint Commission standards related to staff training education established in HR.01.05.03, Element of Performance C24 and align with the Long-Term Care Institute and Center for Medicare and Medicaid Services regulations on memory care. The contractor shall provide Foundation training and re-certify the current Lexington VAMC trainers to teach the One-Day Foundation Course within their organization. Specific training objectives for each day are: DAY 1, June 19th, 2018. FOUNDATION. A Foundation Course shall be taught on the first day. This course shall establish a common framework about Alzheimer s disease and related dementias (ADRD), a best-abilities perspective, dementia stages, behavior management, and care techniques to promote the best ability to function. The course objectives are: Identify empowering paradigm shifts to create successful outcomes for those with dementia. Demonstrate an understanding of Alzheimer s disease and related dementias and their impact on cognitive function. Identify cognitive assessments and screening tools to identify best ability to function. Describe the best ability to function and key remaining abilities for each dementia stage, as defined by Allen Cognitive Levels. Identify key care approaches for each dementia stage to promote the highest possible level of function in ADL, mobility, and leisure activity. Understand strategies to enhance communication, minimize negative behaviors, and support families. DAY 2, June 20th, 2018. INSTRUCTOR COURSE. Shall build on the Foundation course and emphasizes application and include activities, case studies, and discussions relative to assessment, care approaches, documentation, and functional maintenance programs. The course objectives are: Link information about ADRD, cognition, and stages of dementia to work realities. Identify care approaches that will facilitate best ability to function for an individual at each stage of dementia. Practice person-centered communication strategies and care approaches specific to each stage of dementia. Develop customized care strategies. Create a comprehensive care plan, given communication strategies, care approaches, and behavior responses. Explain how various team members efforts contribute to achieving positive outcomes for persons with dementia DAY 3 June 21st, 2018. RE-CERTIFICATION. Shall be the train-the-trainer course, allows an organization to create a positive, congruent approach among all staff. After taking the Two-Day Foundation and Care Partner Applications Course, on day three, participants learn how to deliver consistent staff training rooted in a person-centered, abilities-based model. Upon completion, they become authorized to teach the One-Day Foundation Course within their organization. Anticipated Period of Performance: June 19-21, 2018. Training shall be accomplished at the Lexington VAMC, Lexington, KY Information Sought: Response to this announcement shall not exceed 10 pages on company letterhead and should include the following information: (1) A tailored capability statement clearly indicating the firm s ability to provide the training services described above. Include proof of training and or certification to accomplish the training per The Joint Commission standards related to staff training education established in HR.01.05.03, Element of Performance C24 and align with the Long-Term Care Institute and Center for Medicare and Medicaid Services regulations on memory care. Include references (name, phone number, contract or purchase order number, etc.) if the Dementia training was previously taught; (2) The respondents DUNS number, organization name, address, point of contact, phone number, email address and size and type of business (e.g., SDVOSB, VOSB, small, etc,) pursuant to the applicable NAICS code; (3) Information shall be provided electronically in a Microsoft Word or Adobe PDF format. Please provide the information in one file clearly labelled with the company name and "36C24918Q0320". E-mail your response with the above information to david.milner@va.gov with the subject line "Sources Sought Notice # 36C24918Q0320 by April 23, 2018, 12 pm Central Time.
