- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. Solicitation number 36C24818Q0614 is issued as a Request for Quotation (RFQ). This document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-97, effective 01-24-2018. This RFQ is a 100 % Service Disable Veteran Owned Small Business Set-Aside. The associated North American Industry Classification System (NAICS) Code for this acquisition is 334310, size standard 750 employees. Description of Requirement: The contractor shall be responsible for the installation of Government Furnished Equipment - Public Address (PA) system with the ability to broadcast and be heard over the James Haley VA Hospital campus. Line Item 1: Public Address System Installation Qty: 1, Unit of Issue: JB, FOB: Destination A list of contract line item number(s) and services to be rendered, and projected period of performance can be found on Attachment A, as supplemented by the Performance Work Statement (PWS) found at Attachment B. All services are to be performed and accepted at the Department of Veteran Affairs (VA), James A Haley Veteran s Hospital, 13000 Bruce B Downs, Tampa, FL 33612. The following Provisions applies to this acquisition: Provision 52.212-1, Instructions to Offerors Commercial Items. Offerors must submit their technical and management approach that addresses all requirements pertaining to the required services. Provision 52.212-2, Evaluation Commercial Items. The comparative evaluation procedures will be used in accordance with FAR 13.106-2(b)(3). The Government will use the following criteria for the basis of award: Comparative Evaluation using Price and Past Performance as the factors. Provision 52.212-3, Offeror Representations and Certifications Commercial Items. To be eligible for award offerors must be registered in System for Award Management (SAM) prior to award in accordance with the provision at 52.212-3, Offeror Representations and Certifications -- Commercial Items completed. The following Clauses applies to this acquisition: The clause at 52.212-4, Contract Terms and Conditions -- Commercial Items, applies to this acquisition and no additional addenda to the clause included The clause at 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders -- Commercial Items, applies to this acquisition. See Attachment D for clauses cited in the clause that are applicable to this acquisition. Additional contract requirement(s) or terms and conditions necessary for this acquisition and consistent with customary commercial practices are as follows: Attachment D: FAR 252-2 and VAAR 852.219-10.
