This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation Number: 36C25718Q0442 Solicitation Type: Request for Quotation (RFQ) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-97 11-06-2017. This acquisition is 100% set-aside for Service Disabled Veteran Owned Small Business (SDVOSB) Concerns that have been verified by the U.S. Department of Veterans Affairs Center for Veterans Enterprise (CVE) in accordance with 38 CFR § 74 VA Veteran-Owned Small Business Verification Guidelines and appear in the Vendor Information Pages (VIP) database found at http://www.vip.vetbiz.gov/. The North American Industry Classification System (NAICS) Code associated with this procurement is 337127 Institutional Furniture Manufacturing and the small business size standard is 1,250 employees. Please refer to the attached document Statement of Work for all line items required and scope of work. All items shall be shipped Free on Board (FOB) Destination by in accordance with statement of work. North Texas Veterans Healthcare System 4500 S. Lancaster Road Dallas, TX 75216 Please mark all shipments to the attention of Marcus Crawford, marcus.crawford@va.gov. The provision at FAR 52.212-1, Instructions to Offerors Commercial Items applies to this acquisition. Addenda are attached to the provision. The provision at 52.212-2, Evaluation Commercial Items, will not be used. The comparative evaluation procedures will be used in accordance with FAR 13.106-2(b)(3). See attachment Basis for Award All offerors are advised to include a completed copy of the provision at FAR 52.212-3, Offeror Representations and Certifications Commercial Items with their quote. The clause at FAR 52.212-4, Contract Terms and Conditions Commercial Items applies to this acquisition. Addenda are attached to the clause. The clause at FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders Commercial items applies to this acquisition.
