Description
Sources Sought Synopsis: The 413th Contracting Support Brigade, Regional Contracting Office, Hawaii, is performing market research through this Sources Sought inquiry for a firm fixed price type contract. This is NOT A SOLICITATION for proposals, proposal abstracts, or quotations. The purpose of this notice is to obtain information regarding: (1)The availability and capability of qualified business sources able to perform the required work at Tripler Army Medical Center. (2)What is the size of the capable business, and to identify whether they are large businesses or small businesses: HUBZone small businesses; service-disabled veteran-owned small businesses; 8(a) small businesses; veteran-owned small businesses; woman-owned small businesses; or small disadvantaged small businesses; (3)Their size classification relative to the North American Industry Classification System (NAICS) code for the proposed acquisition. NAICS: 325120 Small Business Size Standard: 1,000 employees. Requirement: Contractor shall provide and deliver Dry Ice to the Department of Pathology. Contractor shall provide Dry Ice cut in blocks 5x10x3/4 inches in Poly bags, with a maximum of 400 lbs. each week. Delivery amount will be determined and communicated by COR to Contractor. Contractor shall provide a maximum of 20,800 lbs. of dry ice per Period of Performance. Contractor shall deliver shipment to Tripler Army Medical Center, Dept. of Pathology Services, Shipping Lab, RM# 2G-008. MCHK-DP every Monday morning, no later than 0700 on the first working day of the week. If there will be a delay in delivery, communication must be made to the Shipping Department at 433-6044 Anticipated Period of Performance: Base Year: 1 July 2018 - 30 June 2019 1st Option: 1 July 2019 - 30 June 2020 2nd Option: 1 July 2020 - 30 June 2021 3rd Option: 1 July 2021 - 30 June 2022 4th Option: 1 July 2022 - 30 June 2023 Submission of Capability Statement /Information Sought: Interested, qualified and equipped organizations should submit a tailored capability statement for this requirement: •1. Not to exceed 2 single-sided pages that clearly detail the ability and equipment available to perform the requirements of this notice in a quick time frame within the next two weeks. •2. On top of the first page of your capability statement should include: •- Indication of current certified small business status or indication of a large business •- Organizations name •- Point of Contact •- Address •- DUNS number •- Cage Code Information Submission Instructions: All capability statements sent in response to this SOURCES SOUGHT notice must be electronically (via email) to SSG Lawes, Christiaan at: christiaan.d.lawes.mil@mail.mil by 25 May 2018 12:00 pm HST. All response must be received by the specified date due date and time in order to be considered. ANY RESPONSES RECEIVED AFTER THE SPECIFIED DATE/TIME WILL NOT BE CONSIDERED.
