Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation VA250-17-Q-0707 is being issued as a Request for Quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-95 01/19/2017. The NAICS code that applies is 515210 and business size is $38,500,000.00. This action will result in a FFP purchase order, utilizing simplified acquisition procedures under Full and Open Competition. Requirement is for Satellite Television services in accordance with attached Performance Work Statement. The Period of Performance (PoP) is a Base plus four (4) Option years (five-year total contract length), exercising of options is dependent on satisfactory service and need for the service. Site visit attendance is highly encouraged and is currently: TBD. The location of site visit will be: Dayton VA Medical Center, 4100 W. 3rd St., Building 320 and 330, Dayton, OH 45428-9000. All interested parties need to ensure they provide list of personnel who plan to attend site visit, submitted to the Contract Specialist via email (Jennifer Bowden, jennifer.bowden@va.gov) no later than 12:00 noon EST, 30 June 2017. All personnel will be escorted from front desk, before proceeding to work site locations. Evaluations factors will be Lowest Price, Technically Acceptable (LPTA). Technical Evaluation will consist of: Price Technical Previous Contracts of similar magnitude One page Quality Control Plan All questions are due no later than 23 June 2017 (15:00 EST). If all answers to questions submitted within this timeframe are not provided at least two (2) business days prior to bid due date, bid due date will be extended accordingly. All answers to questions (including any additional documentation provided) are distributed on a non-attribution basis.
