- ATTACHMENT #3, CONTRACTOR INFORMATION AND PRICING ATTACHMENT #2, FULL TEXT PROVISIONS AND CLAUSES ATTACHMENT #1, WASHER AND DRYER SPECS This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice.This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation, W912LP-18-Q-6003, is being issued as a Request for Quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-97, effective 24 Jan 2018. This procurement is 100% Small Business set a-side. The NAICS code is 333318 and the small business size standard is 1,000 employees. Basis for award is Price. The following commercial items are requested in this solicitation: 0001 - Commercial Washing Machine, 1 each 0002 - 2-year service and maintenance agreement for Commercial Washing Machine, 1 each 0003 - Commercial Dryer, 1 each 0004 - 2-year service and maintenance agreement for Commercial Dryer, 1 each 0005 - Startup and training on Commercial Washing Machine and Commercial Dryer, 1 each SEE ATTACHMENT #1, Washer and Dryer Specs, for the minimum requirements for the above items. Quote Submission Requirements : Contractors shall complete ATTACHMENT #3 and return it with their quotation. Contractors shall provide product specifications/cut sheets with sufficient detail for the government to determine if quoted products meet the minimum requirements, warranty descriptions, and descriptions of the maintenance agreements/service plans that are available for the quoted equipment. Quotes are due at 11:00 AM central time on 14 June 2018, at USPFO for Iowa, Camp Dodge, ATTN: Vicky Williams, 7105 NW 70th Ave, Bldg 3475, Johnston, IA, 50131-1824. Quotes may be submitted by mail to the above address, uploaded to FBO, or e-mailed to vicky.l.williams10.civ@mail.mil. Facsimile quotes will not be accepted. Regardless of method of submission, contractors are responsible for ensuring that quotes are received by the date and time indicated. Late quotes will not be accepted.
