Description: Page 2 of 2 Request for Information UCIS Avaya/Nortel National Consolidated PBX Maintenance Introduction This is a request for information (RFI) only. Do not submit a proposal. It is requested that all companies interested in participating in this effort please note their interest and provide indication of their respective capabilities to perform the effort described in attached DRAFT Performance Work Statement (PWS). This RFI is for planning purposes only and shall not be considered an Invitation for Bid, Request for Task Execution Plan, Request for Quotation or a Request for Proposal. Additionally, there is no obligation on the part of the Government to acquire any products or services described in this RFI. Your response to this RFI will be treated only as information for the Government to consider. You will not be entitled to payment for direct or indirect costs that you incur in responding to this RFI. This request does not constitute a solicitation for proposals or the authority to enter into negotiations to award a contract. The information provided may be used by the VA in developing its acquisition strategy and PWS. Interested parties are responsible for adequately marking proprietary, restricted or competition sensitive information contained in their response. The Government does not intend to pay for the information submitted in response to this RFI. 2. Submittal Information: This is RFI only. It is requested that all companies interested in participating in this effort note their interest and provide indication of their respective capabilities to perform the effort described in the draft PWS. Marketing material will not be accepted as an indication of capability. Provide, at a minimum, the following information in the initial paragraph of the submission: Name of Company Address Point of Contact Phone Number Fax Number Email address Company Business Size and Status NAICS code(s) Socioeconomic data Data Universal Numbering System (DUNS) Number Provide a summary of your capability to meet the requirements. In your summary you must provide a response to the following: Has the draft PWS and associated documents provided sufficient detail to describe the technical requirements to be performed under this effort. ______ YES _______ NO Please provide your technical questions/comments/recommendations on elements of the draft PWS that may contribute to a more accurate proposal submission and efficient, cost effective effort. Please provide your questions/comments/recommendation of elements of the Section B that may contribute to a more accurate cost proposal submission. Small businesses should include intent and ability to meet set-aside requirements for performance of this effort, if applicable. Please provide a Rough Order of Magnitude (ROM) pricing. All responsible sources may submit a response in accordance with the following: No more than 10 pages (excluding transmittal page and ROM) Include the name, email address and phone number of the appropriate representative of your company.
