Description

This is a combined This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 -- Streamlined Procedures for Evaluation and Solicitation for Commercial Items, in conjunction with FAR -- Part 13 Simplified Acquisition Procedures and supplemented with additional information included in this notice. This announcement constitutes the only solicitation; offers are being requested and a written solicitation is attached to this announcement. This solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular 2005-97, dated January 24, 2018. Solicitation # 36C26218Q0646 is issued as a Request for Quote (RFQ) and constitutes the only solicitation. The Government intends to award a single Firm-Fixed Price (FFP) award from this solicitation. The North American Industry Classification System (NAICS) Code is 561790 and the Small Business Administration s (SBA) size standard is $7.5M for this procurement. This procurement will be issued as a 100% total set aside for Service-Disabled Veteran-Owned Small Businesses (SDVOSBs), see FAR Clause 52.219-27 and VAAR Clause 852.219-10. The Government is seeking a Contractor to furnish all equipment, parts, tools, labor, supervision, transportation and travel related costs for the windows and hallway leak repair (caulking) services. This procurement will apply the Service Contract Act Wage Determinations WD 15-5613 (Rev.-9 dated 01/10/2018 for this service requirement. Place of Performance: Tibor Rubin Medical Center, VA Long Beach Healthcare System (VALBHS) located at 5901 E. 7th Street Long Beach, CA 90822. Period of Performance: Approximately June 4, 2018 to November 30, 2018. Provisions / Clauses: The following provisions / clauses: 52.212-1, Instructions to Offerors Commercial, 52.212-2, Evaluation Commercial Items, 52.212-4, Contract Terms and Conditions - Commercial Items, and 52.212-5 - Contract Terms and Conditions Required to Implement Statutes or Executive Orders -- Commercial Items are applicable to this acquisition. Please see the attached document, S02 RFQ 36C26218Q0646 Windows and Hallway Repair Caulking Service, to view Solicitation for the Statement of Work (SOW), additional clauses / provisions, and other pertinent information relative to this solicitation offerors shall review and be aware of. Site Visit: Attendance is NOT mandatory, but highly encouraged so offeror(s) can inspect the site(s) where services are to be performed and to satisfy themselves regarding all general and local conditions that may affect the cost of contract performance, to the extent the information is reasonably obtainable. This will be the only site visit provided by the Government. Offerors shall be responsible and shall not be reimbursed by the Government for any costs or travel expenses incurred related to the site visit. A site visit is scheduled for May 23, 2018 at 10:00 AM PST. All offerors will meet at VA Long Beach Tibor Rubin Medical Center located at 5901 E. 7th Street Long Beach, CA 90822. Offeror(s) will meet with Hyacinthe Luna, Contract Specialist in front of the fuel point by Facilities Management and Engineering in building 5 and beside the VA Police building. Parking is limited and it is suggested offerors who plan to attend to plan accordingly. Interested parties should sign up in advance to attend the site visit by notifying the Contract Specialist Hyacinthe Luna via email: hyacinthe.luna@va.gov no later than 11:00AM PST on May 22, 2018. The email must include the following: Subject of email: RFQ # 36C26218Q0646 VALBHS Windows and Hallway Leaks Repair (Caulking) Services Site Visit Request; Body of email: Company Name; Point of Contact, phone number and email address; and List of all attendees (no more than 2 attendees per company). Basis of Award: Award will be made to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government. The Government s intent is to award without discussions. Price is the only factor that will be used to evaluate offers (see FAR Provision 52.212-2). Award will be made to the lowest priced offer that is determined to be reasonable. After making selection of the lowest price offeror, the Contracting Officer will determine if the selected offeror is responsible. In addition to the General Standard of Responsibility found in FAR 9.104-1 and in accordance with FAR 9.104-2, the following Special Standard of Responsibility apply to this procurement: The Offeror should indicate that it is an offeror for a Government contract when contacting the entity designated at www.sam.gov for establishing the unique entity identifier. Unless exempted by an addendum to this solicitation, by submission of an offer, the offeror acknowledges the requirement that a prospective awardee shall be registered in the SAM database prior to award, during performance and through final payment of any contract resulting from this solicitation. If the Offeror does not become registered in the SAM database in the time prescribed by the Contracting Officer, the Contracting Officer will proceed to award to the next otherwise successful registered Offeror. Offerors may obtain information on registration and annual confirmation requirements via the SAM database accessed through https://www.acquisition.gov. Specialized requirement: Contractor and/or contractor s employee(s) performing on this contract shall have the technical knowledge and experience for the window reseal and repair services. Qualified contractors do not need particular certificates but have adequate working knowledge and has ability to stop windows leaks and caulk the necessary areas. Submitting Offer: Offeror(s) are reminded to thoroughly review the solicitation in its entirety and shall submit their offer using the Price/Cost Schedule found in the attached document under Section B.4. Offeror is advised the Government may be unable to receive other types of electronic files (e.g. compressed or zip files) or files larger than ten (10) megabytes (MB). It is the offeror s responsibility to ensure all required documents are included and completed as required by this solicitation. Offeror(s) shall submit their quotes to Hyacinthe Luna via email at hyacinthe.luna@va.gov.
