Description
THIS IS A SOURCES SOUGHT NOTICE ONLY. THIS IS NOT A REQUEST FOR QUOTES. This sources sought is issued for the sole purpose of conducting market research in accordance with the Federal Acquisition Regulation (FAR) Part 10 to determine if there are a reasonable number of interested business sources capable of satisfying the agency's requirement. The Government is seeking to identify business sources under North American Industry Classification System (NAICS) 621111 (Offices of Physicians, (except mental health) (size standard-$11,000,000). If your firm, regardless of size, is capable of providing these services, please respond appropriately. ****THIS PROCUREMENT IS UNRESTRICTED 1. Contract Information: The Vicksburg Contracting Office (VCO) is soliciting Capability Statements and Past Performance information from all qualified and interested contractors to participate in a new contract requirement for physician services for the Army Corps of Engineers, Vicksburg District. The contract award will include a base year, plus 4 option years. 2. Project Information: The requirement is for physician services on-site at the U.S. Army Corps of Engineers Vicksburg District Headquarters (MVK) located at 4155 E. Clay Street, Vicksburg, Mississippi. In addition to providing on-site physician services, there is an additional requirement to conduct annual physicals for temporary employees for the MAT Sinking Unit (MSU) working season. The intent is to send new employees to the Contractors Health Clinic(s), where they will absorb them into their regular clinic operation. Physicals and blood work are required each week and paper work to include test results shall be available for pick up the following Monday by the MVK Safety Office. Health Clinic shall have the operational capability to provide physical exams for up to 100 employees but not to exceed 200 employees. Government documents are provided to physicians for use during physical examinations. A detailed explanation of government documentation will be provided by a government representative. a. On-site Hours and Performance: Services shall be provided Tuesday and Thursday at MVK, for four hours per day from 08:00am to 12:00pm, Federal Holidays excluded. Days and times of service may be changed by written memorandum of agreement between the parties without modification to the contract. The physician may occasionally be contacted by the Government at other times for medical guidance or assistance during normal business hours. Services will not be required during any period when the location is closed, for example, during inclement weather or on a locally observed holiday. • Equipment: The contractor shall provide all personnel, equipment and supplies needed to give physicals. • Physical Exam Requirements: Conduct pre-employment physical examinations to 100 up to 200 employees per year that will consist of a basic physical that includes at least the following:  Measuring height and weight  Eye exam, according to supplied examination form, section 2 of Part C  Other findings, according to supplied examination form, section 4 of Part C, to include:  Eyes, ears, nose, and throat  Abdomen  Head and back (including face, hair, and scalp)  Peripheral blood vessels  Speech  Extremities  Skin and lymph nodes  Heart  Back (especially for positions involving strenuous duties)  Neurological (including reflexes) and mental health  EKG for employees over 40 years old  Urinalysis U/A- Dipstick only  Blood work consisting of the following:  CBC (complete blood count)  CMP (comprehensive metabolic panel)  Lipid Panel 3. Contractor Personnel Minimum Qualifications: Physicians performing under this contract shall be licensed medical doctors who are qualified to practice in the Vicksburg, Mississippi, area. They shall be graduates of a medical school approved by the Council on Medical Education and Hospitals of the American Medical Association. Additionally, all physicians performing under this contract shall have education, training, or experience either in preventive medicine or in occupational health. 4. Submission Requirements: The Government invites contractors with the capabilities necessary to meet or exceed the stated requirements, as indicated in this posting, to submit a Capability Statement consisting of appropriate documentation, literature, brochures, and past performance information with references. Past performance documentation shall include contact names, as well as a description of the services provided, contract type (i.e., fixed price or cost), and dollar value. Proof of Business Size must also be included. The company's CAGE Code and DUNS number shall also be included with the submission. Contractors MUST have an active record in the System for Award Management (SAM) database. Not having an active record in SAM may result in not receiving consideration. The Capability Statement for this sources sought is not expected to be a proposal or quote, but rather short statements regarding the company's ability to demonstrate existing or developed expertise and experience in relation to similar requirements. Any brochures or currently existing marketing material may also be submitted with the Capabilities Statement. Please indicate your interest and/or ability to provide all services, and capabilities of your facility. This sources sought is for information and planning purposes only and is not to be construed as a commitment by the Government. The Government will not pay for any information received from potential sources as a result of this sources sought. Respondents will not be notified of the Sources Sought results. All interested firms with 621111 as an approved NAICS code have until Friday, June 8, 2018, 10:00 am (central time) to submit the following information: 1. Please provide your company name, address, point of contact with corresponding phone number, e-mail address, CAGE code and small business size under NAICS 621111. 2. Clearly confirm that your company business size as Large or Small Business, and if applicable, designation as HUBZone, 8(a), Service Disabled Veteran Owned Small Business, Small Disadvantaged Business, Woman-Owned Business. 3. Evidence of capabilities to perform comparable/relevant services (not more than five years old). Include location where services were performed; completion date, total contract value, and your company's level of involvement in the services. Please provide at least one point of contact (with email address and phone number) for project validation. 4. What does your company specialize in? 5. Firm's capability to perform, to include geographic location, Medical equipment list and equipment capability? 6. What experience does your company have with providing physician services to other Government entities or contracts? Submissions that are not received within the allotted time and/or do not meet all requirements will not be considered. Electronic submissions via email will be accepted. No hard copy or facsimile submissions will be accepted.
