- In accordance with FAR 5.201, this synopsis has been posted because exercising Option Period (2) two will increase the contract value to $31,500.00 which exceeds the threshold in FAR 5.201(b)(1)(ii) and FAR 5.101(a)(1). HT0011-16-P-0013 Description. The National Quality Forum (NQF) was created in 1999 by a coalition of public- and private- sector leaders after the President's Advisory Commission on Consumer Protection and Quality in the Healthcare Industry concluded that an organization like NQF was needed to promote and ensure patient protections and healthcare quality through measurement and public reporting. The Department of Defense (DoD) was a founding member of NQF and has been a member since its inception if 2000. NQF is the only nationally recognized consensus-based healthcare organization in the nation as defined by the Office of Management and Budget. This status allows the Federal Government to rely on NQF-defined measures or healthcare practices as the best, evidence-based approaches to improving care. The Federal Government, states, and private-sector organizations use NQF's endorsed measures, which must meet rigorous criteria, to evaluate performance and share information with patients and their families. A request for written quote was submitted to NQF on September 27, 2016. An award in the amount of $10,500.00 was made to NQF on a sole-source basis on September 29, 2016. The procurement was not synopsized prior to award. The base period of performance was September 29, 2016 - September 28, 2017 in the amount of $10,500.00. Option Period (1) one was exercised in the amount of $10,500.00 for the period of performance of September 29, 2017 - September 28, 2018. The contract value was increased to $21,000.00 after Option Period (1) one was exercised. This is an annual service contract with one-year annual NQF membership and four one-year membership option periods. The dates of performance are as follows: Base: September 29, 2016 - September 28, 2017 Option 1: September 29, 2017 - September 28, 2018 Option 2: September 29, 2018 - September 28, 2019 Option 3: September 29, 2019 - September 28, 2020 Option 4: September 29, 2021 - September 28, 2022 Under the authority of 41 U.S.C. 1905 and in IAW FAR 13.106-1(b)(1)(i), the contracting officer solicited from one source because it was determined that the circumstances of the contract action deem only one source reasonably available. This procurement was not set-aside for any small business concerns.
