Description

Request for Quotation This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6 Streamlined Procedures for Evaluation and Solicitation for Commercial Items, as supplemented with additional information included in this notice. This procurement is being conducted under FAR Part 13, Simplified Acquisition Procedures. This announcement constitutes the only solicitation, therefore, a written solicitation will not be issued. The Indian Health Service, Yakama Indian Health Center intends to award a fixed price purchase order in response to request for quote #2018-019-1050EQ. See attached PDF. This is a full and open competition. The solicitation documents and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-95. The associated NAICS code is 339115, Ophthalmic Goods Manufacturing, size standard is 500 employees. PRICE SCHEDULE - SEE ATTACHED. PURPOSE OF THE CONTRACT: This requirement is to provide a mobile vision screener with printer and labels to the Yakama Indian Health Center Optometry Department. Please refer to the attached Request for Quotations for details. INSTRUCTION TO OFFERORS: The intent is to award a fixed price purchase order resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be the best value and most advantageous to the Government, price and other factors considered. Basis for award is all or none. Your quote must include all line items. If it does not, it will not be considered. Reference RFQ 2018-019-1050EQ on all submissions. Please include your company DUNS and tax ID number on your quote, as well as an estimated delivery date after receipt of order. Offerors are to email complete quote to prudence.yellowowl@ihs.gov no later than 12noon PST, Monday, August 6, 2018.
