Combined Synopsis/Solicitation for Commercial Items This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 -- Streamlined Procedures for Evaluation and Solicitation for Commercial Items, in conjunction with FAR -- Part 13 Simplified Acquisition Procedures and supplemented with additional information included in this notice. This announcement constitutes the only solicitation; offers are being requested and a written solicitation is attached to this announcement. This solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular 2005-97, dated January 24, 2018. This solicitation, #36C26218Q9703, is a request for quotations (RFQ) and constitutes the only solicitation.The Government intends to award a single Firm-Fixed Price (FFP) award from this solicitation. The associated North American Industrial Classification System (NAICS) code for this procurement is 541990, with a small business size standard of $15.0M. This procurement will be issued as a service-disabled veteran-owned small business (SDVOSB) 100% set-aside, see FAR Clause 52.219-27 and VAAR Clause 852.219-10. All interested companies shall provide offers for the following: The Government is seeking a Contractor to inspect and certify portable fire extinguishers. See Statement of Work attached to this announcement. Place of Performance: The VA Greater Los Angeles (CVA GLA) 3 campuses: West Los Angeles Medical Center 11301 Wilshire Blvd Los Angeles, CA 90073 Sepulveda Ambulatory Care Center 16111 Plummer Street Los Angeles, CA 91343 Los Angeles Ambulatory Care Center 351 East Temple Street Los Angeles, CA 90012 Period of Performance: Tentatively August 15, 2018 to September 30, 2018. The following solicitation provisions apply to this acquisition: 52.212-1, Instructions to Offerors Commercial, 52.212-2, Evaluation Commercial Items, 52.212-3, Offerors Representations and Certifications Commercial Items Nov 2017. The following FAR and VAAR contract clauses apply to this acquisition: 52.212-4, Contract Terms and Conditions - Commercial Items, and 52.212-5 - Contract Terms and Conditions Required to Implement Statutes or Executive Orders -- Commercial Items, 52.217-8 -- Option to Extend Services, 852.203-70 Commercial advertising., 852.232-72 Electronic submission of payment requests, 852.237-70 Contractor responsibilities. Basis of Award: Award will be made to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government. The Government s intent is to award without discussions. Lowest price is the only factor that will be used to evaluate offers (see FAR Provision 52.212-2). Award will be made to the lowest priced offer that is determined to be reasonable. After making selection of the lowest price offeror, the Contracting Officer will determine if the selected offeror is responsible. In addition to the General Standard of Responsibility found in FAR 9.104-1 and in accordance with FAR 9.104-2, the following Special Standard of Responsibility apply to this procurement: Specialized requirement: Contractor and/or contractor s employee(s) performing on this contract shall be able to provide storage, retrieval/refiling and destruction services of non-record lab specimen compliant with OSHA, JC and VA standards/protocols. Submitting Offer: Offeror(s) are reminded to thoroughly review the solicitation in its entirety and shall submit their offer using the Price/Cost Schedule found in section B.3 of the RFQ /SOW document. Offeror is advised the Government may be unable to receive other types of electronic files (e.g. compressed or zip files) or files larger than ten (10) megabytes (MB). It is the offeror s responsibility to ensure all required documents are included and completed as required by this solicitation.
