Description
The Great Lakes Acquisition Center (GLAC) in Milwaukee, WI is searching for vendors with the capability to provide preventative maintenance, software maintenance and updates and service for the Spectrum Dynamics D-SPECT Cardio Camera System Located at the William S. Middleton VA Hospital, Madison, WI. The vendor must be able to meet all the minimum requirements stated in the statement of work (listed below). Interested responsible sources must submit a summary of their capabilities to this office by 0900 CT on Tuesday, August 14, 2018, to include detailed description of the firm s experience in maintaining equipment, DUNS number, name and phone number of a point of contact in case further information is needed. Summaries must be submitted via email to Lori.Eastmead@va.gov. This RFI is issued solely for information and planning purposes to determine availability of qualified vendors and does not constitute a solicitation. All information received in response to this RFI that is marked as proprietary will be handled accordingly. The procurement will be completed under NACIS 811219. Please note that this is not a request for quote and the Government is seeking information for market research purposes only. The Government may or may not issue solicitation documents. Look for any further information in FedBizOpps. Telephone inquiries will not be returned. STATEMENT OF WORK (SOW) D-SPECT Nuclear Scanner Service Contract 1. Scope. The contract shall provide service support if required via Field Service Engineer or Call Center Telephone Support. Biannual preventative maintenance, software maintenance and updates to the Spectrum Dynamics, D-SPECT Camera System, Model D-SPECT Cardio, Serial Number 14251, located in C4109 at the William S. Middleton VA Hospital, Madison, WI. 2. Background. Madison VA's Nuclear Cardiology Lab requires a service contract for the D-SPECT camera system. The complexity of this system, the expense for replacement parts, and essential equipment uptime warrant the need for an active service contract. The quote submitted includes two preventative maintenance tests over the contracted year. These are required to ensure optimal performance. The D-SPECT system is state of art technology. The advanced and sensitive equipment requires handling from only highly skilled individuals. Limited downtime is essential to reduce costs for Non-VA care and postponement of diagnostic studies. 3. Specific Tasks. After initial investigation and repair attempt from Madison VA s Biomedical Engineer Department, Spectrum Dynamics will provide technical support via phone and/or on-site service to resolve issues related to Cardiology s D-SPECT system. Response time from the vendor shall be within one hour for phone support and within 8 hours for on-site support. Costs for equipment repair and shipment expenses shall be included within the cost of the contract. Exclusions are preexisting conditions deemed by the vendor at the initial inspection. Hours for repairmen services are business hours of 0800-1700, Monday Friday. This contract includes biannual preventative maintenance visits. The service contract also includes software maintenance and updates. 4. Performance Monitoring The Government and COR will confirm that any service provided is in compliance with the contract terms. Items monitored will be timeliness of emergent services and repairs for broken equipment. Any variance of the contract will be reported to the CO to document and communicate with the contractor. Biannual preventative maintenance reports shall be given to the COR to ensure completion and standards are met. 5. Security Requirements Contracted employees providing services at VA facilities may undergo a security clearance in the form of a basic background investigation. 6. Government-Furnished Equipment (GFE)/Government-Furnished Information (GFI). The government will not provide any equipment to the contractor. The Contractor will need to supply their own tools for execution of this contract. Contractor shall be responsible for securing contractor-owned and VA-owned equipment at all times. Any lost or stolen equipment must be reported as soon as it is known to the COR. 7. Other Pertinent Information or Special Considerations. The Contractor will be responsible to ensure Contractors employees providing work on this contract are fully trained and completely competent to perform the required work a. Identification of Possible Follow-on Work. Service contract has been renewed annually due to complex equipment. Option years to extend contract versus beginning new annual contracts are desired. b. Identification of Potential Conflicts of Interest (COI). No conflicts of interests are known. c. Identification of Non-Disclosure Requirements. Contractor will be exposed to PHI information. No additional risks are assumed. d. Packaging, Packing and Shipping Instructions. No unique requirements are identified. e. Inspection and Acceptance Criteria. The COR will certify that the work done under the contract is performed to time and standard. The COR will also assure the inspection and acceptance of products provided incidental to services. 8. Risk Control Contractor will be exposed to PHI information. No additional risks are assumed. 9. Place of Performance. Work is to be completed at the William S. Middleton Memorial VA Hospital, 2500 Overlook Terrace in Madison, WI 53705. Associated travel expenses to service the equipment are included within the service contract.
