THIS IS A SOURCES SOUGHT NOTICE ONLY. This notice does NOT constitute a request for proposal, request for quote, or invitation for bid. No formal solicitation document exists at this time. Hampton Veterans Affairs Medical Center (VAMC) Network Contracting Office 6 is conducted market research to find vendors that are capable, willing, and certified. We are trying to determine whether to set-aside an acquisition to provide ambulance services at the Fayetteville VAMC located at 2300 Ramsey St Fayetteville, NC 28301. The scope of the acquisition includes but is not limited to: Provide Basic Life Support (BLS) and/or Advanced Life Support (ALS) Ambulance Services. The contractor shall furnish twenty-four (24) hour ambulance service for the beneficiaries of the Department of Veterans Affairs Medical Center (VAMC), Fayetteville, North Carolina and Fayetteville VA Health Care Center (HCC) located at: Surrounding Fayetteville area Fayetteville VAMC 2300 Ramsey St Fayetteville, NC 28301 Fayetteville HCC 7300 S. Raeford Road Fayetteville, NC 28304 Please provide the following: If your company is interested in performing these services, please notify our office of your intent. In response, please include the following: (a) A positive statement of your intent to submit a bid as a prime contractor (capability statement); (b) Evidence of experience in work similar in type and scope to include c) If any teaming or subcontracting possibilities are contemplated, please provide subcontractor s name and breakdown of labor and its performance cost contract numbers, project titles, dollar amounts, points of contact and telephone numbers; (d) Past performance references with points of contact and phone numbers. (e) At least two (2) references are requested, but more are desirable. Responses to this notice should also identify whether or not the firm falls into any of the socioeconomic categories listed above.
