- This is a SOURCES SOUGHT ANNOUNCEMENT ONLY. It is neither a solicitation announcement nor a request for proposals or quotes and does not obligate the Government to award a contract. Requests for a solicitation will not receive a response. Responses to this Sources Sought must be in writing. The purpose of this Sources Sought Announcement is for market research to make appropriate acquisition decisions and to gain knowledge of potential qualified businesses interested and capable of providing the requested commodities. The NAICS Code is 337214; size standard is 1000 Employees. 100% VOSB set-aside. Responses to this notice shall include the following: (a) company name b) address (c) socio-economic status, (d) point of contact (e) phone, fax, and email (f) DUNS number (g) Cage Code (h) Tax ID Number (i) and must provide a capability statement that addresses the organizations qualifications and ability to perform as a contractor and provide the commodities described below. The Government is not obligated to nor will it pay for or reimburse any costs associated with responding to this sources sought synopsis request. This notice shall not be construed as a commitment by the Government to issue a solicitation or ultimately award a contract, nor does it restrict the Government to a particular acquisition approach. The Government will in no way be bound to this information if any solicitation is issued. Notice to Potential Offerors: All Offerors who provide goods or services to the United States Federal Government must be registered in the System for Award Management (SAM located on the web at www.sam.gov). It is desirable that any Offeror to have completed their business Online Representations and Certifications Application (ORCA). Additionally, all Service Disabled Veteran Owned Small Businesses or Veteran Owned Small Businesses who respond to a solicitation on this project must be registered with the Department of Veterans Affairs Center for Veterans Enterprise VetBiz Registry located at http://vip.vetbiz.gov. Requests for information and resultant responses shall be submitted directly to the contracting officer, Wilmen Joa at Wilmen.Joa@va.gov. NO TELEPHONE CALLS WILL BE ACCEPTED. All submissions should be received no later than 11:00 am Eastern Standard Time on August 14, 2018. Statement of Work (SOW) Scope of Work The procurement is to purchase and install furniture at the New York Harbor Healthcare System Brooklyn Campus, Mental health Clinic and Mental Health Clinic Waiting Area Item/Quantity Description Technical Specification (Detailed description) QTY two seaters OFS Voyage - Abacus ABA 17 Malibu - Model 5220WA-CA separated by arms 3 Individual OFS Voyage - WHN 12 Warm Sand - Model 5220WA-1 9 Individual - PET 101 Springtime - Model 5220WA-1 9 Individiaul OFS Voyage - AC-62897 Seafoam - Model 5220WA-1 18 Installation Contractor will be responsible for equipment delivery and installation. Place of Performance / Delivery Equipment is to be delivered to the warehouse/loading dock at each campus. Below are the addresses for both campuses. NYHHCS Brooklyn Campus Warehouse: 800 Poly Place Brooklyn, NY 10010 Installation Location: Delivery VA NYHHS Brooklyn Warehouse Travel/Parking The contractor will be responsible for their travel. As stated earlier, due to other construction projects and limited parking available at the site, contractor must coordinate any parking needs with the assigned COR and the Police Service. Details of any vehicles entering the site must be provided in advance. (Driver s Full Name, Driver s License Number, Vehicle Description, Vehicle License Plate Number) Government-Furnished Property Contractor will not need any government furnished equipment or property. Education and Training Requirements The Vendor shall be responsible for ensuring that vendor employees providing work on the required equipment are fully trained and completely competent to perform the required work. Specify what training and education is required for the clinical staff to be competent in using/operating the supply. Recalls The Vendor shall immediately notify all affected VA Medical Centers and designated POC in the event of a device recall, removal, required adjustment, or required modification that is suggested or mandated by the Vendor, distributor, manufacturer, or any regulatory or government agency. The following information shall be included in this notification: A complete item description and identification (to include lot and serial numbers where applicable); Administrative identification data including order numbers and order date; Reasons for recall, removal, adjustment, or modification; and Instructions for appropriate corrective action. Defective Items/Warranty All furniture should come with at least a 1 year warranty time period from the day of installation. Any defective items must be replaced ASAP.
