All information received in response to this RFI that is marked as proprietary will be handled accordingly. In accordance with FAR 15.201(e), responses to this notice are not offers and cannot be accepted by the Government to form a binding contract. Equipment being ordered: Heated Mobile Plate and Bowl Dispenser Ice cream Freezer Reach-in Refrigerator (Sm.) Reach-in Refrigerator (Lg.) Transaction number: 644-18-3-201-0893 Please provide the following information clearly and precisely. The Phoenix VA Health Care System is looking to procure a Heated Mobile Plate and Bowl Dispenser, Ice Cream Freezer, Reach-in Refrigerator (Sm.), Reach-in Refrigerator (Lg.); their intention is to replace equipment that is beyond its End of Life Cycle and deemed beyond economical value to repair. A list of salient characteristics is provided to discern whether your product can meet the needs of the facility requirement. The applicable NAICS code is 335220 Major Household Appliances Manufacturing. Small Business manufacturers for this NAICS code have a size standard of 1,500 employees. The resulting contract will be a firm fixed price order. The term of the contract is intended to be 30 calendar days ARO, with no options included. It is anticipated that a Request for Quotes will be issued August 15, 2018. Award of a firm fixed price contract is contemplated by August 31, 2018. This notice is published to conduct market research to determine if there are a sufficient number of certified Service Disabled Veteran Owned Small Businesses (SDVOSB), Veteran Owned Small Businesses (VOSB), Small Businesses (SB), or Large Businesses capable of performing the requirements to warrant a socio-economic set-aside. Interested parties responding to this sources sought request shall submit the following information at a minimum: (1) company name and address, point of contact with phone number, and DUNS number; (2) list items to offer as open market or Federal Supply Schedule with contract number; (3) socio-economic status with proof of registration in the VA Vet Biz if SD/VOSB. Indian School Road, Room 1153, Phoenix, AZ 85012. The Contractor shall be responsible for assessing the Medical Center for all requirement cabling and other equipment needed for installation. The contractor will disassemble and remove the old machine to the dock, uncrate and set up the new machine in place, and assemble the new unit and make all internal connections. The contractor will re-connect the new unit to the existing utilities and perform the start-up check to ensure proper function. The Contractor will consult and coordinate with the in-house engineering group to gain access to restricted areas. All installation plans will be reviewed and signed off on by the project COR, requestor and engineering representative. Warranty: The contractor shall provide the standard 12 or 24 months warranty; with an option to purchase an extended warranty of 3-5 years that will cover all parts, labor, supplies, travel costs, and user support at no additional cost to PVAHCS. Operating Constraints: All installation and implementation work must be completed after regular business hours and on weekends due to infection control concerns for installation during food preparation. The hours for installation will be coordinated/agreed upon prior to installation. PVAHCS must have discretion to defer/hold any installations, if locations are not accessible for the installation process.
