Description

These are the provisions and clauses for the solicitation and anticipated purchase order. This is the Statement of Work (SOW) for this requirement. This is a Request for Quotation (RFQ) with anticipated CLIN structure for the purchase order. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Requirements (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The solicitation number for this requirement is 2018-26527 and is hereby issued as a Request for Quote (RFQ) using FAR Subpart 13.1 Simplified Acquisition Procedures. The Centers for Disease Control & Prevention (CDC) intends to award a fixed-price purchase order for this requirement. All work shall be performed according to the attached Statement of Work (SOW). Your quote shall include the following: * Total price for CLINs 0001-0005 and 1002-1003. * Capabilities in accordance with the SOW and evaluation factors. * Cost breakdown for price(s) offered. Cost breakdown should be in sufficient detail for the Government to perform cost and/or price analysis. Please include any documentation of authorized indirect labor rates. Period of Performance: Period of Performance is twelve (12) months from the date of award. Delivery Address: Primates shall be delivered to CDC's loading dock. Centers for Disease Control & Prevention (CDC) 1600 Clifton Road NE Building 23 Atlanta, GA 30329 Acceptance and FOB point. The quote format is at the discretion of the Offeror. It is the Offeror's responsibility to be familiar with the applicable clauses and provisions. Clauses and provisions may be accessed via the Internet at website https://www.acquisition.gov/. This solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular (FAC) 2005-87-2 (6 April 2016). This acquisition is solicited under North American Industry Classification System (NAICS) code 112930 and small business size standard of tiny_mce_marker.75M. A fixed-price purchase order will be issued in writing to the successful Offeror. To be eligible to receive an award resulting from this solicitation, the Contractor must be registered in the System for Award Management (SAM). Offerors may register in SAM at http://www.sam.gov. For questions regarding registration, contact Federal Service Desk at (866) 606-8220. Include FAR 52.212-3, Offeror Representations and Certifications with Offer or indicate if it is available online at http://www.sam.gov. No telephonic quotes will be processed. All electronic responses must be received by the Government no later than 10:00am ET on Thursday, 16 August 2018. An electronic quote shall be emailed to the Contracting Officer, and a copy of the quote on compact disc (CD) shall be sent to the Contracting Officer at the following address. The CD shall be received by the Government no later than two (2) business days after Thursday, 16 August 2018.
